The mid-dle incisural space is a narrow cleft between the upper brainstem medially, at the level of the pontomesencephalic junction, and the medial aspect of the temporal lobe, namely the parahippocampal gyrus.
It corresponds to the ambient and crural cisterns.
The part of the middle incisural space exposed by this approach corresponds to the trigonum lemnisci and to the segment (area) of the cerebellomesencephalic fissure occupied by the superior cerebellar peduncle and by the dorsal portion of the middle cerebellar peduncle.
