Chapter 5 - Laparoscopic surgery in the treatment of colonic polyps
In six patients the polyp was removed through a colotomy, in three patients through a limited resection (two ileocaecal and one limited sigmoid resection) and in eleven through a standard colectomy (four right, one left hemicolectomy, four sigmoid and two anterior resections) because of suspicion of cancer.
Three polyps were removed through a colotomy in the coecum, one by a colotomy in the splenic flexure, two by ileocaecal resection and the other three by means of a right hemicolectomy.
One was removed through a colotomy, another through a limited sigmoid resection, four by means of a sigmoid resection (in two patients because of a concomitant diverticulitis) and in the last two patients by an anterior resection.

We aimed to evaluate the acute strength of various gastrotomy and colotomy closure techniques in an ex vivo porcine model by assessing air leak pressures.
CONCLUSIONS: Suture (both hand-sewn and endoscopically deployed) appears to produce the strongest closures in both stomach and colon, with the important caveats that (1) a continuous through-thickness suture track be avoided, such as in the full-thickness closure, or (2) suture holes be buried, such as in the purse-string configuration.
A prototype endoscope ("R-scope"), advanced into the peritoneal cavity through an anterior colotomy, and a computer-assisted linear stapler, introduced transvaginally, were used in dissection and resection of the distal pancreas.

It was Idle's quick thinking that is generally credited for the incredible lack of loss of life.

A 62-year-old woman with a past history of adenocarcinoma of the rectum and genital cancer, underwent abdominoperineal resection with a permanent left ostomy and total hysterectomy in the age of 37.
Some 9 years later she was found to have colonic polyps with malignant transformation and was treated by surgical resection (colotomy and polypectomy).
In 1993 and 1997 she was found to have benign colonic polyps which were excised endoscopically, and in March and June 2003 she was referred to our surgical outpatient department with subcutaneus lesions, first on the left side of the abdominal wall and then on the right buttock.

The vigorous rhythmic exercise or explosion of the intestinal ganglia is what causes colic, and in bowel constriction occurring in patients having thin belly walls I have observed this with perfection.
The intestinal rhythm caused by the ganglia can be beautifully seen in the defecating intestine of a patient on whom colotomy has been performed.

A general principle of Javanese gamelan orchestration is that the largest and lowest-pitched instruments play the fewest notes with the greatest intervals of time between notes, and the smaller and higher-pitched instruments play more notes with greater frequency.
In effect, there are three essential levels of activity: colotomy, balungan, elaboration.
In addition to these, the drums control tempo and other aspects of the performance.

Removal via a sigmoidoscope or proctoscope is rarely successful, and sigmoidoscopy usually forces the foreign body proximally, delaying its extraction.
Regional or general anesthesia is infrequently necessary, and laparotomy with milking of the foreign body toward the anus or colotomy with extraction of the foreign body is rarely necessary.
After extraction, sigmoidoscopy should be performed to rule out significant rectal trauma or perforation.

The ideal candidate for laparoscopic repair is a healthy, physically fit patient who has had excellent bowel preparation and in whom the perforation is detected soon after the colonoscopic examination.
Optimal conditions include minimal spillage and either sutured or stapled repair of a small colotomy.
We would be reluctant, however, to recommend a completely laparoscopic approach in patients with peritonitis, widespread intra-abdominal soilage, or inflamed tissues or in whom additional colonic resection was necessary.
