 Posterior Abdominal Wall - Lymph
The Common Iliac nodes then drain into the Lumbar or Lateral Aortic nodes found on either side of the Aorta.
These nodes drain into the Common Iliac nodes found close to the common iliac arteries.
When observing the major lymph nodes from inferior to superior in the posterior abdominal wall, one can first label the nodes near the external and internal iliac arteries as External and Internal Iliac nodes respectively.

The Circulatory (Part 3) and Lymphatic System
Find the right and left internal, external, and common iliac arteries, the right and left external and common iliac veins, the abdominal aorta, and the inferior vena cava.
Identify abdominal aorta, celiac trunk (cut), superior mesenteric artery (cut), right and left renal arteries, inferior mesenteric artery, right and left common iliac arteries, right and left common iliac veins, inferior vena cava (cut), and renal veins (cut).
Trace lymph from the foot though lymphatic stuctures (important nodes and stuctures) until it reaches the circulatory system.

eMedicine - Lymph Node Dissection, Retroperitoneal : Article by Jong M Choe, MD, FACS†
The retroperitoneal lymph nodes that are irradiated by this method include the ipsilateral external iliac, bilateral common iliac, paraaortic, and paracaval nodes.
The testicular lymphatics of the left testicle drain stepwise from testis to the nodes of the left paraaortic, preaortic, left common iliac, left external iliac, interaortocaval, precaval, and finally, to the paracaval nodes.
Lymph nodes of the testis extend from T1 to L4 but are concentrated in the region of the renal hilum due to their common embryologic origin with the kidney.

Squamous Cell Carcinoma of the Ovary - A Rare Entity
Left common iliac and internal iliac lymph nodes 2cm x1cm.
Pelvic and paraaortic lymph nodes were not palpable.
No evidence of squamous cell carcinoma was seen in the specimens received including lymph nodes.

 Endoscopic lymphadenectomy
Common iliac lymphadenectomy is indicated in cases where the interiliac lymph chain has been invaded or to analyse the common iliac nodes.
She indications for peri-aortic lymphadenectomy are cervical, endometrial or other pelvic cancer where there is suspicion of CT scan of lymph node invasion with negative cytology or "tru-cut" probe biopsy.
The indications for laparoscopic pelvic lymphadenectomy are lymphatic exploration of pelvic cancers: cervical and endometrial cancer and laparoscopic preparation of extended radical vaginal hysterectomy (Shauta-Dargent technique).

LYMPHATICS OF ABDOMEN, PELVIS, PERINEUM AND LOWER EXTREMITY
Lymph from the bladder goes to the internal iliac, common iliac then para-aortic nodes.
Lymphatic drainage of the abdomen, pelvis, perineum and lower extremity tends to follow blood vessels and ligaments and passes through small clusters of nodes located on viscera or at bifurcations of vessels before draining into groups of larger nodes along major vessels.
Lymphatic drainage follows the same path as other structures in the perineum to the superficial inguinal nodes.

Inferior Vena Cava: is formed on the right side of L5 by the union of the two common iliac veins, below the bifurcation of the aorta.
Lymphatic drainage: intermediate colic lymph nodes, then the inferior mesenteric lymph nodes
The inferior gluteal artery is a branch of the anterior division of the internal iliac artery, it leaves the pelvis through the inferior part of the greater sciatic foramen, goes inferior to piriformis muscle and serves the muscles and skin of the inferior buttock and posterior thigh.

The retroperitoneal lymph nodes that are irradiated by this method include the ipsilateral external iliac, bilateral common iliac, paraaortic, and paracaval nodes.
Postchemotherapy RPLND uses full bilateral dissection of retroperitoneal lymphatics (see Images 5-6) with mobilization of both the aorta and vena cava in their entirety.
With better understanding of the retroperitoneal lymph node anatomy and of lumbar sympathetic nerve distribution, the surgical boundaries of traditional RPLND have been reduced.

eMedicine - Cervix, Cancer : Article by Faysal A Saksouk, MD
The cervix has a rich network of lymphatics that drain principally into the paracervical lymph nodes and subsequently to the hypogastric and external iliac nodes (of which the obturator nodes are the inner most component).
In the US: In the United States, invasive cancer of the cervix is the third most common genital malignancy in women, after cancers of the endometrium and the ovary.
The intravenous injection of contrast material is particularly useful in increasing the conspicuity of the cervical tumor and in facilitating the evaluation of the parametria and the pelvic sidewalls.

Conventional surgery + Lymphadenectomy: as conventional surgery plus systemic dissection of the iliac and obturator nodes.
In 1996 a paper by Thomas and Blake, highlighted the uncertainty surrounding the roles of pelvic lymphadenectomy (removal of lymph nodes in the pelvis whether or not diseased) and radiotherapy following surgery in the treatment of early stage endometrial cancer.
A randomised trial to determine the impact of lymphadenectomy and of adjuvant external bean radiotherapy in the treatment of endometrial cancer, high risk pathology and no macroscopic disease following surgery.
