Because tissue perfusion in a compartment is dependent on arteriolar and capillary perfusion gradients, a compartment syndrome may result in the presence of peripheral pulses.
It is important to avoid the superficial peroneal nerve during its exit from the fascia of the lateral compartment traversing anterior in the distal third of the leg.
The lateral compartment is then released with an incision on the inferior-medial aspect of its overlying fascia, extending to the lateral side of the foot.

Compartment syndrome   (Site not responding. Last check: 2007-10-08)
The compartment syndrome is a condition in which increased pressure within a muscle compartment causes a decrease in blood supply to the affected muscles.
The so-called compartments are groups of muscles surrounded by inelastic fascia and thus any swelling of muscles leaves no room for expansion and blood supply is progressively cut-off.
The chronic compartment syndrome which occurs as an overuse injury is most commonly seen in the muscles of the lower extremity but has also been described in the forearm and the thigh.

Compartment syndrome is a general term which can cover a variety of different areas in the body but for this discussion we're going to focus on the anterior compartment of the lower leg as this is the primary area that would be affected in runners and triathletes.
This anterior compartment is defined by the tibia bone on the inside, the fibula bone to the rear, and the crural fascia surrounding it all.
By definition, compartment syndrome "occurs in anatomic locations that have unyielding, well defined osteofascial spaces."(1) That makes the anterior compartment of the lower leg the perfect place for potential problems because it is bordered on 2 sides by bone.

Acute and chronic compartment syndromes are discussed and differentiated, and the importance of a high index of suspicion in all cases is emphasised.
Acute compartment syndrome is of particular concern because the diagnosis must be made essentially on clinical grounds and must be acted upon promptly if serious and potentially irreversible injury to the relevant compartment is to be avoided.
Literature reports suggest that the anterior compartment of the lower leg is the most susceptible and that this may be because the fibrous and osseous boundaries are far more defined and rigid than in the other compartments.8 Ger et al.

The Physician and Sportsmedicine: Chronic Exertional Compartment Syndrome
Chronic exertional compartment syndrome (CECS), though less serious, is painful and persistent and may halt physical activity, so primary care physicians should be able to recognize CECS in athletes.
All compartments can be safely and efficiently evaluated in an outpatient setting, but knowledge of anatomy is essential for identifying the appropriate compartment and avoiding neurovascular structures.
The first step in compartment pressure measurement is, of course, to identify the compartment and sterilize the overlying skin.

Compartment syndrome involves the compression of nerves and blood vessels within an enclosed space, leading to impaired blood flow and nerve damage.
Compartment syndrome may also occur due to tight bandages or casts; with significant swelling, pressure will build up and can cause compartment syndrome.
Chronic compartment syndrome can be caused by repetitive activities like running that increase the pressure in a compartment only during that activity.

Compartment syndrome (CS) is a condition in which the perfusion pressure falls below the tissue pressure in a closed anatomic space with subsequent compromise of circulation and function of tissues.
The compartments of the lower leg and the volar forearm are particularly prone to developing elevated compartment pressures.
In cases of suspected CS, immobilize tibial fractures with the ankle in slight plantar flexion, which decreases the deep posterior compartment pressure and does not lead to an increase in the anterior compartment pressure (note that postoperatively the ankle is held at 90 degrees to prevent equinus deformity).

The hallmark symptom of compartment syndrome is severe pain that does not respond to elevation or pain medication.
When the compartment pressure is greater than 45 mmHg or when the pressure is within 30 mmHg of the diastolic blood pressure (the lower number of the blood pressure), then the diagnosis is made.
When chronic compartment syndrome is suspected, this test must be performed immediately after the activity that causes pain.

Acute Compartment Syndrome - DynoMed.com   (Site not responding. Last check: 2007-10-08)
Acute compartment syndrome is also called Volkman’s ischemic contracture.  Ischemic contracture, which occurs with the passage of time, means the muscles that are not getting enough blood begin to tighten, contract, and die.
In most cases, a trauma or injury is to blame for acute compartment syndrome.  A broken or crushed leg or arm can put pressure on nerves or muscles in the area.  And the injury itself can block the flow of blood to the area.  With little blood flow, the cells begin to die.
A less common cause of acute compartment syndrome can be a greatly increased level of exercise.  People new to the military or people who run long distances may be at risk.
