Heart block - Wikipedia, the free encyclopedia
Blocks that occur within the sinoatrial node (SA node) are described as SA nodal blocks.
Blocks that occur within the atrioventricular node (AV node) are described as AV nodal blocks.
This is because if an individual had complete block at this level of the conduction system (which is uncommon), the secondary pacemaker of the heart would be at the AV node, which would fire at 40 to 60 beats a minute, which is enough to retain consciousness in the resting state.

Complete atrioventricular block, shock, and hyperkalemia induced by toxic adenoma of the thyroid gland.
Complete heart block in thyrotoxicosis is uncommon and previous reports relate to its occurrence in Graves' disease, usually complicated by infectious disorders.
We conclude that the varying degrees of heart block were secondary to hyperthyroxinemia caused by Graves' disease and exogenous thyroid hormone.

Heart block is a disorder of the heartbeat.
This interrupts the passage of impulses from the atria to the ventricles.
Heart block may be caused by coronary artery disease, inflammation of the heart muscle, rheumatic fever, or overdose of certain heart drugs.

 Heart Block
Heart Block is a type of bradycardia (too-slow heartbeat) that also is called atrioventricular, or AV block.
In this condition, the electrical signals that stimulate heart muscle contractions are partially or totally blocked between the upper chambers (atria) and the lower chambers (ventricles).
Complete Heart Block may be a medical emergency with potentially severe symptoms and a serious risk of cardiac arrest (sudden cardiac death, or SCD).

Radiation damage to the valves and conducting system of the heart is rare and although both have been described in isolation, the case we describe is unique in that both complications have occurred in the same patient.
The valvular involvement followed the heart block by a number of ears, suggesting that after irradiation a sequence of cardiac complications may occur since different cardiac structures degenerate at different speeds.
In only one other case was heart block associated with the treatment of carcinoma of the right breast.[12] There was associated pericardial disease in ten patients, and right ventricular outflow tract stenosis was seen in 2.[19,24] None of these papers reported coexistent valvular dysfunction.

Conduction abnormalities are also not uncommon, with transient left bundle-branch block being reported in 13 percent[3] and transient CHB in approximately 1 percent of the cases.[11] To our knowledge, this is the first report of ABV-induced persistent heart block requiring a permanent pacemaker.
Heart block requiring permanent pacing is a recognized complication of aortic valve ring abscess and aortic valve replacement.

When a child has heart block, the electrical impulse is delayed or blocked completely as it travels from the heart’s upper chambers (the atria) to the heart’s lower chambers (the ventricles).
Heart block occurs at the level of the AV node.

Not all patients with AV dissociation have complete heart block.
Pathophysiology: Third-degree heart block is caused by conduction block at the level of the AV node, the His bundle, or the bundle branch-Purkinje system.
When the block is at the level of the AV node, the escape rhythm generally arises from a junctional pacemaker with a rate of 45-60 beats per minute.

Autoimmune congenital heart block is considered, but not definitively proven, to be the result of transplacental passage of maternal antibodies (ie, alloantibodies), which affect atriventricular conduction tissues.
Congenital heart block is presumed to be caused by injury from the placental passage of maternal anti-Ro (or related) antibodies, which are present in more than 90% of mothers if tested during pregnancy or at the time of delivery.
Children with isolated congenital complete heart block are most likely to have acquired the disease as a complication of subclinical or clinical maternal lupus erythematosus, maternal Sjögren syndrome, or another maternal autoimmune disease.

If however there is bundle damage the escape rhythm is generated lower down the conducting system resulting in a slow and unreliable heart beat and life- threatening asystoles.

Questions in The Heart Forum are being answered by doctors from The Cleveland Clinic Heart Center, consistently ranked the #1 Heart Center in America.
CHB or complete heart block means that the heart beat that originates in the sinus node (found in the upper chambers of the heart) does not get transmitted to through the AV node to the ventricles (lower and main pumping chambers.)
The lower chambers therefore rely on their intrinsic rhythm to beat and at best this rate is 45 bpm and certainly not enough to allow for exertion, sometimes it is so slow that the person can not even stand!
