I received 'conditional' baptism in 1979 in an ELCA congregation--but in a Baptist immersion tank, as I wanted immersion, and was instructed 'it was the Word with the water', not the immersion,that saved.
We prefer not to use the term "conditional baptism" because there was nothing conditional about the nature of your baptism.
Calling the baptism "conditional" was not a statement about the nature of this baptism, but only a statement that the church was uncertain whether you had ever been baptized and was baptizing you to remove any uncertainty.

A conditional baptism was given to Carl at Saint Boniface Church, 124 Locust Street, Buffalo, New York, on September 6, 1887 --- perhaps as part of a conversion to the Roman Catholic Church.
A baptismal record for Bernhard Otto Rebescher lists both Carl and Veronika as sponsors of this baptism in Buffalo on February 21, 1897, indicating that Carl was still alive as of that date.
