eMedicine - Papilloma, Conjunctival : Article by Hon-Vu Q Duong, MD
Conjunctival papillomas are categorized into infectious (viral), squamous cell, limbal, and inverted (histological description) based on appearance, location, patient's age, propensity to recur after excision, and histopathology.
The limbal conjunctival papilloma often is referred to as noninfectious conjunctival papilloma because it is believed that limbal papillomas arise from UV radiation exposure.
Egbert et al reported a case of conjunctival papilloma in an infant born to a mother with HPV infection of the vulva during pregnancy.

  Common Infections
Patients with bacterial conjunctival infections typically present with injection of the bulbar and palpebral conjunctiva and a papillary response on the superior palpebral conjunctiva.
Conjunctival injection, superficial punctate keratitis, superior corneal pannus, peripheral subepithelial infiltrates, iritis and follicles (most dense in the inferior cul-de-sac) may all be present.
Viral conjunctival infections are thought to be transmitted by airborne respiratory droplets or direct transfer from one's fingers to the conjunctival surface of the eyelids.

Conjunctival tumors usually first appear as a spot or slightly raised area against the white background of the eye and are diagnosed by an eye doctor using a slit lamp.
Conjunctival melanoma appears as a brown or pink nodule on the white surface of the eye and carries a 20 percent risk for metastasis.
Conjunctival tumors are often diagnosed by examination with a slit-lamp biomicroscopy.

Conjunctival pigmented lesions, termed conjunctival lentigines by dermatologists, can be seen in two familial lentiginosis syndromes: in approximately one quarter of patients with Carney's complex, sometimes in association with eyelid myxomas, and in Peutz-Jeghers syndrome.
Conjunctival epithelium may be tangentially sectioned (particularly if the biopsy was not fixed flat), may appear polypoid in plica and fornices, or may exist as conjunctival cysts or downward epithelial proliferations; PAM with atypia affecting such epithelium may give the false impression of melanocytic cells involving substantia propria.

Conjunctival flap appearance can vary significantly between subjects in both the extent (length of the conjunctival flap area) and the width (Figure 3) of the flap tissue.

The flap may be present by itself or in combination with conjunctival staining and indentation (which may be the early development stage of the flap).

Conjunctival tumors may first appear as a pink or brown bump on the eye.
Depending on the specific type it is, conjunctival tumors have a relatively low incidence of metastasis, but it is vitally important that it be caught early so as to prevent it from invading the orbit and progressing into malignant melanoma.
Surgery to remove the tumor is the treatment of choice for most conjunctival tumors although the patient may need additional treatment with cryotherapy (a freezing technique) and surgery on the sclera and epithelium.

Conjunctival malignant melanoma is a rare malignancy of the conjunctiva, the eye's outermost covering.
A conjunctival melanoma cannot be diagnosed with an eye examination.

The responding physicians observed conjunctival hyperemia in 11.3 percent (±15.2) of patients treated with latanoprost, 30.3 percent (±24.7) with bimatoprost and 22.6 percent (±23.3) with travoprost (P=0.001).* When present, the average severity of redness was: 1.5 (±0.6) for latanoprost; 2.5 (±0.8) for bimatoprost; and 2.1 (±0.8) for travoprost.
This survey indicates that conjunctival hyperemia, both in incidence and severity, appears to be highest with bimatoprost and lowest with latanoprost, but may be noticed with all three prostaglandin-related medicines.
Direct, quantitative, prospective comparative trials are needed to evaluate the clinical impact of conjunctival hyperemia in patients with primary open-angle glaucoma or ocular hypertension.

The conjunctival flap is a well-proven, time-honored treatment for numerous disparate comeal diseases sharing in common a persistently compromised ocular surface.
The purpose of a conjunctival flap is to restore the integrity of a chronically compromised comeal surface, typically damaged because of neurotrophic or neuroparalytic disease, severe dry eye, or bullous keratopathy.

Conjunctival autograft was obtained from upper temporal limbus and secured with 10-0 monofilament nylon.
The mean size of the pterygium was 3.80 mm (range 2.6 to 4.8 mm) in the mitomycin-C group and 3.60 mm (range 2.5 to 4.5 mm) in the conjunctival autograft group.

Foreign bodies may become trapped behind the nictitating membrane and induce conjunctival irritation and inflammation, and ulceration of the ventronasal cornea.  Always evaluate behind the nictitating membrane after topical anesthetic is instilled.  Foreign body should be removed using fine forceps and treatment with a broad-spectrum topical and oral antibiotic is indicated.
Movement or protrusion of the nictitating membrane is passive in the dog.  Protrusion occurs secondary to retraction of the globe into the orbit which causes forward displacement of orbital fat and protrusion of the membrane.
