The creek is formed in Franklin County, Pennsylvania at the confluence of the West and East Branches of Antietam Creek about 2.3 miles (3.7 km) south of Waynesboro, Pennsylvania.
The creek is noted for numerous well preserved stone arch bridges dating to the 19th Century that still traverse the creek, the most famous of which is the 125 foot (38 meter) long Burnside Bridge in the Antietam National Battlefield.
The creek was a major topographic feature during the Battle of Antietam (called the Battle of Sharpsburg in the American South) fought on September 17, 1862, near Sharpsburg, Maryland.

Connoquenessing Creek water quality will eventually improve and Zelienople's water supply will have added protections under a new consent agreement between the U.S. Environmental Protection Agency and AK Steel.

AK Steel has indicated it is considering supplying the borough with a mobile reverse osmosis filtration system if the creek water must be used this summer, but a final decision hinges on the borough receiving a state discharge permit for the system.

Slippery Rock Creek flows in a deep, winding gorge cut into flat-lying layers of bedrock.
This is not a typical pattern because the valleys of streams in areas of flat-lying rocks typically become progressively shallower and wider toward their outlets.
Today, Slippery Rock Creek still has a steep gradient through the gorge (28 ft per mile compared to 8 feet per mile north of Kennedy Mill).
