Some who claim to be root cause analysis experts say that fixable has no place in the definition of a root cause or in evaluating the goodness of a root cause analysis tool.
Therefore, the investigator must know all possible causes and must do some sort of "validation" (is the cause necessary and sufficient) of the root cause analysis to PROVE that the cause they selected produced the effect they observed.
They miss the lessor contributors that may have cause the fire to be worse or caused response to the fire to be slow.

Some people seem to think that the only way to arrive at a root cause (or root causes) for an accident is to do original thinking for each investigation.
Thus to have this common characteristic of a system (and be repeatable), a root cause analysis tool must be well thought out and overcome a common human tendency to jump to conclusions and to only collect evidence that verifies the investigator's hypothesis.
