Cor pulmonale - Wikipedia, the free encyclopedia
Cor pulmonale is a medical term used to describe a change in structure and function of the right ventricle of the heart as a result of a respiratory disorder.
Cor pulmonale may lead to congestive heart failure (CHF), with worsening of respiration due to pulmonary edema, swelling of the legs due to peripheral edema and painful congestive hepatomegaly.
CHF is a negative prognostic indicator in cor pulmonale.

 Cardiology Advisor 2005.2: Cor Pulmonale
Cor pulmonale is enlargement of the heart's right pumping chamber (right ventricle) due to high blood pressure in the lungs.
Cor pulmonale usually occurs late in the course of lung disease.
Less commonly, cor pulmonale may be caused by small blood clots that repeatedly come from veins lower in the body and lodge in the lung blood vessels.

THE MERCK MANUAL, Sec. 16, Ch. 203, Heart Failure
Cor pulmonale is directly caused by alterations in pulmonary circulation that lead to pulmonary arterial hypertension, thereby increasing the mechanical load on RV emptying (afterload).
Cor pulmonale should be suspected in all patients with one of the underlying causes (see Etiology, above).
In cor pulmonale due to disease of the pulmonary parenchyma, clinical manifestations of the primary disease frequently overshadow those of cor pulmonale.

Cor pulmonale was defined in 1963 by the World Health Organization as "hypertrophy of the right ventricle resulting from diseases affecting the function and/or structure of the lungs, except when these pulmonary alterations are the result of diseases that primarily affect the left side of the heart, as in congenital heart disease."
The diagnosis of cor pulmonale was established by ECG in 43 patients (78.18%) and by echocardiography in 57 patients (95%).
Finally, cor pulmonale is associated with increased mortality and risk of hospital readmission, and influences the length of hospital stay during acute exacerbations of COPD.

Cor pulmonale is an increase in bulk of the right ventricle of the heart, generally caused by chronic diseases or malfunction of the lungs.
Cor pulmonale, or pulmonary heart disease, occurs in 25% of patients with chronic obstructive pulmonary disease (COPD).
The prognosis for cor pulmonale is poor, particularly because it occurs late in the process of serious disease.

Cor pulmonale literally means “the heart of the lungs.” Its formal pathological definition is “right ventricular enlargement, hypertrophy, or dilation, secondary to lung disease.” To understand cor pulmonale, one must appreciate the physiology of the normal pulmonary circulatory system and its response to disease.
Time and experience have shown that digitalis is not effective in the treatment of cor pulmonale when it is used without first controlling the patient's underlying hypoxemia and pulmonary hypertension.
Digitalis is a poor inotropic agent in cor pulmonale, but it still has a role as a chronotropic drug since it helps control ventricular rates in patients with atrial fibrillation or flutter and a fast ventricular response.

Cor pulmonale is difficult to diagnose on clinical grounds because signs and symptoms are late and non-specific (5).
As Fishman notes, after age 50 cor pulmonale is the most common cardiac disorder except for coronary and hypertensive heart disease and most of these instances (of cor pulmonale) are secondary to obstructive disease of the airway and subsequent pulmonary hypertension that follows.

 MEDSTUDENTS-PNEUMOLOGY
Cor pulmonale can occur acutely as a result of a single large embolus or chronically from the cumulative effect of multiple small emboli.
the presence of hypotension from acute cor pulmonale nearly always means that the right ventricule is dilated and overfilled and is working against an extremely high afterload and triscupid regurgitation, and any effort to increase flow is met with a dramatic rise in pulmonary arterial and right ventricular pressures.
Clinically, acute cor pulmonale with hypotension looks a little like hypovolemic shock, but with respect to the Starling curve, it is just the opposite, and agressive fluid challenge often pushes a dilated right ventricule into complete collapse.

 Cor pulmonale
Cor pulmonale is failure of the right side of the heart caused by prolonged high blood pressure in the pulmonary artery and right ventricle of the heart.
Normally, the left side of the heart produces a higher level of blood pressure in order to pump blood to the body; the right side pumps blood through the lungs under much lower pressure.
When this right ventricle fails or is unable to properly pump against these abnormally high pressures, this is called cor pulmonale.

Cor pulmonale   (Site not responding. Last check: 2007-10-21)
Cor pulmonale is enlargement of the right side of the heart, which pumps blood to the lungs.
This enlargement occurs when the heart has to work harder to pump blood through damaged lungs, such as when a person's lungs are damaged because of chronic obstructive pulmonary disease (COPD).
People with cor pulmonale are short of breath because they get too little oxygen into their blood.

Cor pulmonale is also called right-sided heart failure, and is characterized by enlargement of the right ventricle.
Treatment targets the underlying illness and may include supplemental oxygen, a low-salt diet or calcium channel blockers.

Objectives: The aim of this study was to assess serum levels of soluble Fas (sFas), an inhibitor of apoptosis, in patients with schistosomal cor pulmonale as compared to patients with cor pulmonale due to chronic obstructive pulmonary disease (COPD) and normal subjects.
Methods: Serum sFas was assessed in 15 men with schistosomal cor pulmonale (age 32 ± 10 years), 15 men with chronic cor pulmonale secondary to COPD and 20 healthy men, matched for age.
Conclusions: Serum sFas levels are elevated in patients with schistosomal cor pulmonale and they are related to the severity of pulmonary hypertension.
