In the immediate aftermath the focus of the investigation is on the condition of points at the crossover known as Lambrigg, with early indications that these were the "immediate cause of the derailment".
A bar designed to keep the switch rails (which move to enable trains to divert or join between two routes) at the correct distance apart was missing, and bolts that secured another two bars were not in place.
The marks on the crossover indicate that at the time of the derailment the train wheels were set on a course where the gauge was narrowing as the train moved forward.

There was no indication that the east crossover switch point rail had been forced open by any wheel flanges, and the connecting rod from the switch points to the switch stand was not bent in the manner typical of a switch being run through.
The rail was laid on hardwood ties placed at 60 ties per 100 feet (30 m) on crushed rock ballast and secured with four standard track spikes and four standard rail anchors per tie.
The crossover consisted of two No. 12 turnouts, each with a standard radius of curvature of 1,104.63 feet (336.7 m), one to the right and one to the left, and switches at Mile 46.72 in the north track (the west crossover switch) and Mile 46.68 in the south track (the east crossover switch).

The rail was laid on 14-inch double-shouldered tie plates on a mix of treated softwood and hardwood ties, with an average of 55 ties per 100 feet of track.
The guard rail was on the gauge side of the crossover's south rail, approximately 79 feet from the crossover entrance on the north track.
The gouge mark on the guard rail at the initial point of derailment indicates that wheel lift was one of the primary mechanisms involved in initiating the derailment.

When the crossover tube is off put new injectors in the four corner spots in the fuel rails.
To bend the crossover like that I just grabbed it on both sides by the ends that go into the fuel rails and SLOWLY squeezed it together with it bending in the middle at the top.
The crossover tube has to be pretty good too where it mounts to the fuel rails or the gaskets that seal it might end up leaking.

The half fishplate which fell into the four foot side was trapped in the narrow gap between the rails on the apron of the crossing and subsequently lodged in the nose of the crossover.
None of the classic wheel climb and rail head marks were to be seen on the rails, which supports the theory that on striking the broken fishplate, the wheel set was thrown from the track.
Marks in the throat of the crossover were consistent with impact damage from a fishplate being trapped between the casting and the wheel(s).

To build the custom crossover, I started by selecting the inner loop as the pass-thru and I would add the crossover approach line by adding cut tracks on each side where the crossover would exist from the outer loop wye to the turntable.
By hand-placing the tracks around the pass-thru line at the crossover location, I then marked and cut each track; in my case I cut both the end of the outloop’s 072 Wye switch and a 072-radius track that lead to the turntable.
Also cut out the center fl rail on the pass-thru track from one end of the cut made to the other; carefully remove the spikes and fl rail.
