Cryoprecipitate is indicated for bleeding or immediately prior to an invasive procedure in patients with significant hypofibrinogenemia (<100 mg/dL).
Cryoprecipitate should not be used for patients with von Willebrand disease or Hemophilia A (Factor VIII deficiency) unless they do not (or are not known to) respond to DDAVP and recombinant and/or virally inactivated preparations are not available.
Fibrin glue: Although single units of cryoprecipitate are available for use in the preparation of fibrin glue to be applied locally for surgery, commercially available, virally inactivated concentrates have a higher fibrinogen concentration and are preferred for this purpose (Tisseel®).

Cryoprecipitate is a human blood component obtained from fresh frozen plasma (FFP) prepared from a unit of whole blood (WB).
One unit of cryoprecipitate derived from a unit of whole blood contains a volume of 10-20 mL, 80-100 units of factor VIII which consists of both the procoagulant activity and the von Willebrand factor, 150-250 mg of fibrinogen, 50-100 units of factor XIII, and 50-60 mg of fibronectin.
In the past, cryoprecipitate rich in factor VIII and von Willebrand factor, was considered the treatment of choice for factor VIII replacement in Hemophilia A and von Willebrand disease.

Assays were performed on the cryoprecipitate for fibrinogen yield (control: 26.4% ± 5.9%, treated; 97.0 ± 2.1%), fVIII yield (control; 35.4 ± 12.6%, treated; 97.2 ± 2.5%), and the weight of each cryoprecipitate pellet was measured (control: 0.6 ± 0.2 g, treated: 15.0 ± 3.5 g).

A colleague in the Pacific Northwest reports that she is aware of two traditional manufacturing 'formats' for single donor cryoprecipitate and a novel prepooled format which is available in her area.
She is fearful that other blood banks may be using inappropriate saline solutions for preparation of pooled cryoprecipitate, such as the saline used by respiratory therapists or for blood bank serologic testing.
