Three of these patients had cryptomenorrhea while the other 3 had severe coital difficulties.
Despite the fact that normal coitus was impossible in these 3 women, 2 of them achieved successful pregnancies and had to be delivered by emergency cesarean section followed by resection of the occluding membranes.

Cryptomenorrhea is a condition where menstruation occurs but is not visible due to an obstruction of the outflow tract.
Specifically the endometrium is shed, but an congenital obstruction in the vagina or on part of the hymen retains the menstrual flow.

 Is your pain really endoemtriosis?
There will also be amenorrhea if there is no uterus (see WebPage 18) or if the endometrium in the uterus has been destroyed (endometrial atrophy) or replaced with intrauterine adhesions (see WebPage 17).
Cryptomenorrhea, or "hidden menstruation", is apparent amenorrhea, resulting from an obstruction to menstruation such as an incompletely formed vagina or a transverse vaginal septum (see WebPage 18).
Whereas amenorrhea itself is usually harmless and painless, cryptomenorrhea is cyclically painful and can cause complications, including endometriosis, while it's unrelieved.

The most common clinical symptom was cryptomenorrhea (15), followed by pelvic pain (11), palpable abdominal mass (9), urinary retention and other voiding problems (8) and problems of defecation (4).
After hymenectomy, the women with imperforate hymen were markedly relieved of cryptomenorrhea, and problems of micturition and defecation also greatly improved.
Although complaints of irregular menstruation and dysmenorrhea gradually evolved as the center of attention during follow-up, most patients fared well in terms of fertility and sexual function.

Although risk factors for the latter include a menstrual period, menarche in particular is not known to predispose a patient to this event.
Severe unilateral pelvic pain with first menses is more likely to herald a congenital mullerian anomaly and cryptomenorrhea, particularly when accompanied by a pelvic mass.
We present a case of tubal torsion where a coincidental, yet misleading temporal relation to menarche led to a delay in laparoscopy and ultimate diagnosis.

As a diagnostic tool, the use of MRI has increased in recent years, although it should be limited to those patients in whom a definitive diagnosis cannot be ascertained based on conventional HSG, ultrasound, and hysteroscopy findings.
MRI is useful for delineating complex pelvic masses and for helping in the diagnosis of such conditions as congenital malformation related to cryptomenorrhea and absence of the cervix.
Other entities associated with primary amenorrhea include congenital absence of the uterus, vagina, or hymen (cryptomenorrhea) and enzyme deficiencies such as 17-alpha hydroxylase deficiency and 17,20-desmolase deficiency.

A less rare abnormality is vaginal atresia, or closure, an obstruction of the vagina by a membrane just above the level of the hymen; menstruation occurs, but the discharge cannot escape and distends the vagina.
This condition, called false amenorrhea or cryptomenorrhea, is easily corrected by an incision in the membrane.
Cessation of periods after menstruation has been established, but before the normal time for the menopause, is usually the result of some general illness, some emotional disturbance, or a psychiatric illness.
