eMedicine - Dissection, Aortic : Article Excerpt by: John Wiesenfarth, MD, MS, FACEP, FAAEM
Cystic medial necrosis is the hallmark histologic change associated with dissection in those with Marfan syndrome.
Cystic medial necrosis was first described by Erdheim in 1929.
Early on, cystic medial necrosis described an accumulation of basophilic ground substance in the media with the formation of cystlike pools.

The most common cause of obstruction of the cystic duct is from a small gallstone that occludes the lumen of the duct.

 Medial degeneration
Medial degeneration is a degenerative disorder in which elastic tissue and muscle fibres degenerate and various amounts of ground substance are deposited
note that the now defunct term “cystic medial necrosis” is not used for medial degeneration as necrosis is not observed and the cysts are probably areas in which smooth muscle cells or elastic fibres once existed but have disappeared due to degeneration
The best known clinical manifestation of medial degeneration is that which involves the ascending and transverse aortic arch causing anulo-aortic ectasia and aortic insufficiency.

Cystic medial necrosis or degeneration is a disorder of large arteries, in particular the aorta, where there is a focal degeneration of the elastic tissue and the muscle in the media, with the presence of mucoid material in the media.
It is more frequent after 40 years of age and is twice as common in males than in females.

Thoracic aortic aneurysms are being identified more often than in the past because computed tomography (CT) of the chest to screen for other disorders is used more widely.
In a common form of thoracic aortic aneurysm, the walls of the aorta degenerate (a condition called cystic medial necrosis), and the part of the aorta nearest the heart enlarges.
This enlargement may cause a malfunction of the valve between the heart and the aorta (aortic valve), allowing blood to leak backward into the heart when the valve is closed.
