Ulceration is caused by ischemic injury to the gastric mucosa, loss of cytoprotectants and assault by gastric acid.
The result is reduced production of cytoprotectant factors, such as prostaglandins, mucosal atrophy and increased permealility, loss of reparative capacity and loss of ability to neutralize acid (hydrogen ions).
The mucosa is compromised by ischemia and attacked (mostly) by acid.

Until recently, high radiation doses have not been considered feasible or safe for patients with advanced lung cancer.
The drug Amifostine used in the trial is a cytoprotectant which concentrates in the healthy tissue, protecting this tissue from being damaged by radiation or chemotherapy.

SAN ANTONIO-A cytoprotectant reduces salivary problems in head-and-neck cancer patients getting radiation therapy.
In a controlled multicenter study of 303 patients with squamous-cell carcinoma, 51% of 153 patients given IV amifostine (Ethyol, Alza) about 15 minutes before each radiation treatment had grade 2 or higher xerostomia, vs. 78% of those who did not get the drug, says Duke radiation oncologist David Brizel.
Overall survival at two years was 81% for the amifostine patients and 73% for the controls, Dr. Brizel told the American Society of Therapeutic Radiology and Oncology meeting here.

Voltaren (Diclofenac): Voltaren (Diclofenac) is a NSAID (non-steroidal anti-inflammatory drug) that is excluded from the treatment of osteoarthritis when used alone.
In Arthrotec, it is used along with a cytoprotectant called Misoprostol (protects the stomach and duodenum against ulceration), in which case it is an acceptable form of osteoarthritis treatment for those who are at high risk for gastrointestinal ulceration (GI).
It is also important to note that Voltaren (Diclofenac), when compared to other NSAID's, has an increased risk of developing NSAID-induced hepatic injury.
