 Aidsmap | ddC - overview
A randomised study comparing twice-daily ddC- and 3TC-containing triple regimens has found that the two regimens are equivalent in terms of the proportion of patients achieving undetectable viral loads after 48 weeks of treatment, although the time to virological failure was shorter in the ddC group (Antunes 2004).
The commonest side-effects of ddC are diarrhoea, constipation, mouth ulcers, rash, fever, and weakness.
There is also limited evidence that ddC can increase the chance of the development of lymphomas, although it is difficult to distinguish the effects of the drug from those of HIV infection itself.

 (ATN) ddC Background
ddC (2',3'-dideoxycytidine), an experimental antiretroviral, is a nucleoside analog (in the same class of drug as AZT).
ddC, which has been available from chemical supply houses for years (it is used in DNA experiments in laboratories) was one of the first drugs tried as an anti-HIV treatment.
The DSMB found that there were 59 deaths in the 320 patients assigned to ddC, compared to 33 deaths in the 315 assigned to AZT -- meaning that AZT clearly worked better when used as a single drug, at least at the doses being tested.

Clinical Alert: DDI & DDC Therapy for AZT-Intolerant HIV Patients
The study was an open-label comparison of ddI and ddC with progression of disease, including death, and tolerance of the study drugs as the main endpoints.
The results of this study suggest that ddC is an acceptable alternative monotherapy to ddI in patients intolerant of or failing on ZDV; ddC monotherapy delays clinical progression at least as long as ddI, and may provide a survival advantage.
Since the majority of patients (66 percent overall) experienced disease progression with either treatment and the differences in outcome are not large, the toxicity profiles of these two drugs will be important to consider when choosing antiretroviral treatment for an individual patient.
