Parfitt, who was jailed for two years in May 2002 for the violent street muggings of two women, failed 10 drug tests out of 19 after he was placed on a combined tagging, home curfew and drug screening regime.
Parfitt, who once described himself as a "raging crackhead", was not deemed a priority offender by probation staff.
He said Parfitt's original officer was adequately briefed on national guidelines to deal with the drug testing programme, but said she had "failed to absorb" the finer detail.

David Parfitt told Birmingham Crown Court he never had any intention of killing or injuring Nottinghamshire Pc Ged Walker, who died after being dragged 100 yards by a taxi Mr Parfaitt was driving.
Mr Parfitt, of Aspley in Nottingham, denies murdering the 42-year-old officer, who died in hospital two days after striking his head on a concrete bollard as he was thrown clear from the car.
The court heard last week that Pc Walker became "stuck" in the window of the cab as it was driven away from a property in Bulwell, Nottingham, on 7 January.

The report looked into the failures by Nottinghamshire probation service that allowed a drug addict, David Parfitt, who had been allowed out of prison on licence subject to drug testing, to kill PC Ged Walker after flinging him off a stolen car he was driving.
As the Telegraph reports, Parfitt failed ten drug tests and failed to make numerous appointments with his probation officer.
The chief probation officer David Hancock, who said not only would he not resign but that none of his officers would be disciplined, blamed the pressure of work.

Agatha Christie herself would no doubt wish that David Suchet could be adorned with a much more luxuriant moustache than the little "clip on" one seen here.
David Suchet and James Fox are good, but far too many of the rest of the cast act like they're playing bad comedy stereotypes, and the overall direction is 'stagey'.
In an interview recently David Suchet said that these latest Poirot films (Death On The Nile, Five Little Pigs & Sad Cypress) are much darker than the television series he did a few years ago and that they stick much more closely with the original storylines, he is not wrong.

There is apparently a Mohammed Abdel Wahab song in Lami called 'Bortuqal', and you can hear pieces in Maqam Lami on the Munir Bashir CD 'L'Art du 'Ud' and also on the Yair Dalal CDs 'Al Ol' and 'Samar'.
It would be very cool I think to put the various makam (maqam) systems side by side to compare them closely.
David pretty much said it all..I just want to suggest listening to couple of clips on the said maqam.
