Dementia praecox - Wikipedia, the free encyclopedia
Dementia in this older sense meant “incoherence.” The primary disturbance in dementia precox was not one of mood (as was the case in manic-depressive illness), but of cognition.
In this edition dememtia praecox became one of the “endogenous dementias.” It is in the the 1913 third volume (second part) of this edition that Kraepelin adjusts his concept of prognosis to admit that a partial remisison of symptoms occurred in approximately 26 percent of his patient.
This brought dementia praecox in line with Eugen Bleuler’s claims about schizophrenia, which he had insisted from the start (in 1908) that (a) in many cases there was no fateful progressive deterioration, that (b) in some cases the symptoms did indeed remit for periods of time, and (c) that there were cases of complete recovery.

 Eugen Bleuler (www.whonamedit.com)
The older form [dementia praecox] is a product of a time when not only the very concept of dementia, but, also that of precocity, was applicable to all cases at hand.
I call dementia praecox "schizophrenia" because (as I hope to demonstrate) the "splitting" of the different psychic functions is one of its most important characteristics.
Bleuler concluded that the disease was not one of dementia, a condition involving organic deterioration of the brain, but one that consisted of a disharmonious state of mind in which contradictory tendencies exist together.

Kraepelin selected the term “dementia praecox” in order to stress the early mental deterioration which followed the onset of the disorder.
The term “dementia praecox,” however, was originated by Austrian psychiatrist Benedict Augstin Morel (1809-1873) in 1860 to describe a mental and emotional deterioration that had its onset around the time of puberty.
Besides modifying the term “dementia praecox,” Kraepelin described three types of this disorder: the paranoid, the hebephrenic, and the catatonic.

While both terms (dementia praecox and schizophrenia) were in use by the health care professionals of his era, Edgar Cayce consistently preferred the term dementia praecox.

 Constructive Delusions - Delusions
MOST psychiatrists state or tacitly assume that dementia praecox is a disease of a steadily progressive nature, where the first symptom of dementia is a signal for relentless degradation of the patient's mental capacity except in the sphere of the more mechanical, intellectual functions.
We may say that the praecox patient as opposed to the paranoia has a poverty or inappropriateness of affect, a scattering of thought and a lack of systematization in his delusions.
Moreover, by choosing for analysis a case which was neither dementia praecox nor paranoia but a combination of the two, he reaches conclusions which are valuable additions to our knowledge of psychotic processes but merely confuse the issue as to the specific mechanisms of paranoia and dementia praecox.

Bleuler wrote a small volume in defense of Freud's psychoanalysis, which appeared in 1911, by which time Bleuler had already parted company with the movement on the basis of what he considered to be Freud's excessive emphasis on infantile sexuality [Stone, 1997, 147].
He felt that it was misleading both because it implied deterioration, which he did not think was inevitable, and because it suggested the presence of a true dementia like senile dementia, which was not the case.
Kraepelin insisted that Dementia Praecox was a single disease, even though he thought that no single symptom occurred in every case.

As a diagnosis-by-exclusion, schizophrenia must be distinguished from the numerous psychiatric and organic disorders that also can lead to psychotic disturbances in thinking and behavior (see Delirium, Dementia, and Amnesia).
A comprehensive physical examination and laboratory evaluation is required when an organic etiology or drug intoxication may be related to mental status changes (see Delirium, Dementia, and Amnesia).
However, it may be necessary to perform some studies to rule out possible organic etiologies for psychosis or to uncover complications of schizophrenia and its treatment (see Delirium, Dementia, and Amnesia).
