Dera Ismail Khan takes its name from Ismail Khan, a Baluch chief who settled here towards the end of the 15th century, and whose descendants ruled for 300 years.
Dera Ismail Khan District is an administrative district of Pakistan's North-West Frontier Province.
It is bounded on the east by Punjab Province, to the southwest by South Waziristan district, and to the northwest by Tank and Lakki Marwat districts.

To the S. it is bounded by Bluchistan and Dera Ghazi Khan district of the Punjab, on the E. by Kashmir and the Punjab, and on the W. by Afghanistan.
Dera Ismail Khan district is one of the hottest areas in the Indian continent, while over the mountain region to the north the weather is temperate in the summer and intensely cold in the winter.

The town of Dera Ismail Khan was founded on the name of his son Ismail Khan somewhere in the beginning of 16th century.
Dera Ismail Khan was also made the divisional headquarter of the Derajat in 1861 and remained as such till the formation of NWFP as a separate province in November 1901.
The Dera Ismail Khan district since then had been having three tehsils Dera Ismail Khan, Kulachi and Tank till recently wherein 1992 Tank was upgraded to the status of district and Paharpur to a tehsil/sub division in 1995.
