One approach to counting the derangements of n elements is to use induction.

Internal joint derangement has two forms: with reduction (when the disk is anterior to the condyle at rest; the misplaced disk returns to the head of the condyle for at least part of the mouth's opening-closing cycle) and without reduction (when the disk is always anterior).
Derangement without reduction usually produces no sound, but maximum opening between the tips of the upper and lower incisors is reduced to <= 30 mm from the normal 40 to 45 mm.
Derangement without reduction is diagnosed by having the patient open as wide as possible; the physician then exerts gentle pressure to open the mouth a little wider but cannot.
