  Dermatome - Patent 4754756
The dermatome of claim 1 wherein the crosshead means comprises a spring biased between a portion of the body and a portion of the crosshead, the spring being adapted to continuously urge the crosshead toward its said final position.
The dermatome of claim 6 wherein at least one of the pins is undercut in a direction facing away from the other pin and wherein an edge portion of one of the blade openings is positioned within the undercut portion to prevent unintentional blade separation.
The dermatome of claim 11 wherein the crosshead means comprises a detent pivotally affixed to the crosshead, the detent being pivotally movable by the lever toe to permit passage of the lever toe when the operating lever is pivotally moved in a counter-clockwise direction.

Dermatomes (An Overview)   (Site not responding. Last check: 2007-09-18)
Dermatomes are located on the anterior body and the posterior body and are consecutive in the neck and torso regions.
In the appendages, however, adjacent dermatome patterns overlap and the arrangement of the dermatomes is markedly different on the anterior portion of the body as opposed to the posterior aspect.
Because adjacent dermatomes overlap in the arms, legs, groin and buttocks areas, at least three spinal nerves must be blocked to achieve a loss of feeling in these regions.

 Myotomes & Dermatomes dermatome map
A skin area innervated by the sensory fibers of a single nerve root is known as a dermatome.
Dermatome is a Greek word which literally means "skin cutting".
The dermatomes form into bands around the trunk but in the limbs their organisation is more complex as a result of the dermatomes being "pulled out" as the limb buds form and develop into the limbs during embryological development.

A dermatome is an area of the skin supplied by nerve fibers originating from a single dorsal nerve root.
A knowledge of the dermatome map is essential in order to recognize nerve root damage or radiculopathy.
Radiculopathy is suspected when there is loss of sensation in a dermatome.

Did you know that most of the dermatome charts that are floating around the internet and on the walls of doctors' offices were based on investigations made over 35 years ago at best (31), and over 90 years ago at worst(30).
As you can see by these results, S1 dermatomal pain (pain in the side of the foot and little toe) is not very accurate at predicting the level of nerve root compression/irritation.
Although the majority of patients seem to share the same 'nerve root dermatomal distributions' (wiring), this investigation has clearly demonstrated that the neural anatomy of the lumbar spine does have some degree of variation, i.e., some 20% of the patients did NOT have the typical 'nerve root dermatomal distributions'.

The oldest method for determining the dermatome is to follow the distribution of the nerves by dissection.
Shingles (herpes zoster), for example, is a viral infection of the spinal ganglion cells that produces skin eruptions in the dermatome of the affected dorsal roots.
They ignore, for example, the great overlap between neighboring dermatomes, and on the other hand, that the dermatomes are much wider than the zones of hypesthesia occuring after damage to one dorsal root.

Postherpetic neuralgia is defined as pain that persists in the affected dermatome or that appears after the cutaneous HZ eruption has healed (i.e., all the crusts have fallen off; usually 30 days after disease onset) [16].
PHN is the most common complication of HZ: in the absence of antiviral therapy, it affects roughly 10% to 15% of all patients, more than 20% of patients older than age 60, and 50% of patients over the age of 80 [5].
Acute neuritis occurs in 93% of patients with ophthalmic zoster, localized to the affected dermatome and proportional to the severity of the rash.

Shingles (herpes zoster) is a vesicular rash occurring in a sensory dermatome due to the reactivation of the endogenous varicella-zoster virus that has persisted dormant in dorsal root ganglia or trigeminal ganglia since a childhood infection with chickenpox.
The rash of shingles is characteristic and comprises a spotty to continuous erythematous macular rash with clusters of clear vesicles that conform to a dermatomal distribution.
In some patients, the dermatomal pain does not disappear but persists for prolonged periods.26 Dermatomal pain persisting after complete healing of the rash and longer than three months defines postherpetic neuralgia.12,27 Using this definition, the incidence of postherpetic neuralgia in the elderly population is about 10%.

Each dermatome is the area of skin innervated by a certain nerve root.
The dermatome diagram shows all the dermatomes, and makes it possible to understand the otherwise confusing distribution of the dermatomes.

Although this test was performed quickly during a dermatome cutting experiment, a reliable signal was detected such that computer timing issues are now being considered.
In order to better protect the dermatome control circuitry during the dermatome cutting experiments, the dermatome control circuitry was mounted in a separate box as seen in Figures 1 and 2 below.
As far as the actual dermatome skin grafting experiments are concerned, the modified dermatome does produce better grafts than the previous model but it still tends to sink into soft surfaces thereby displacing the cutting angle of the laser.

Based upon this law and supporting EMG studies, we can assume that underlying dermatomes are residing myotomes and sclerotomes with resulting sensory and motor dysfunction.
If the practitioner is attempting to trace dermatome patterns, they should be aware that segmental nerve supply could vary as to site of innervation or supply from the dermatome charts that they are referencing.
Not only can the segmental supply be off by one or two segments, but also the patterns overlap, thus, making objective specificity difficult to interpret.
