MedlinePlus Drug Information: Desflurane (Inhalation-Systemic)   (Site not responding. Last check: 2007-10-19)
Desflurane (DES-flure-ane) belongs to the group of medicines known as general anesthetics (an-ess-THET-iks).
Desflurane is used to cause general anesthesia (loss of consciousness) before and during surgery.
However, desflurane caused other unwanted effects in the animal fetus when given for many days in a row in amounts that were large enough to cause harmful effects in the mother.

Desflurane ( Suprane )   (Site not responding. Last check: 2007-10-19)
Desflurane is resistant to chemical degradation and undergoes negligible metabolism (approximately equal to 10% of that seen with isoflurane).
Desflurane 'wash-out' is approximately equal to 2 to 2.5 times faster than that of isoflurane in the first 2 hours after discontinuation of anaesthesia.
Comparative studies of desflurane and propofol, and other inhalation agents, indicate that the times to toleration of oral fluids, sitting and discharge from recovery room are similar, regardless of the general anaesthetic agent administered.

Desflurane is one of a family of halogenated methylethylethers which is administered by inhalation producing a dose-related, reversible loss of consciousness and of pain sensations, suppression of voluntary motor activity, modification of autonomic reflexes and sedation of respiration and the cardiovascular system.
Desflurane is not recommended for use as an inhalational induction agent in paediatric patients because of the frequent occurrence of cough, breath holding, apnoea, laryngospasm and increased secretions in children under 12 years.
Desflurane appears to produce coronary vasodilation at arteriolar level in selected animal models, in a similar fashion to that of isoflurane.

Desflurane produces concentration related respiratory and cardiovascular depression in spontaneously breathing patients and hypotension in ventilated patients with hypercarbia and hypoxia.
Administration of desflurane should be stopped, the airway should be checked to ensure it is open and assisted or controlled ventilation with pure oxygen should be maintained along with adequate haemodynamics.
Concentrations of 1-4% of desflurane in a mixture of nitrous oxide and oxygen havebeen used successfully in patients with chronic renal/hepatic impairment and duringrenal transplantation surgery.

Desflurane differs from isoflurane by the substitution of a chloride atom with a fluoride atom at the alpha-ethyl component of the molecule.
Desflurane is extremely volatile, a characteristic that necessitated the development of a new vaporizer for its use.
Desflurane causes a dose-dependent increase in heart rate at deep levels while isoflurane increases rate at all levels of anesthesia.

Economic value of Desflurane in comparison to Isoflurane in Low Flow Anesthesia   (Site not responding. Last check: 2007-10-19)
Desflurane is one of the new anesthetics characterized by a low blood solubility, which allows a rapid saturation of the blood.
If Desflurane is used in high fresh gas flows, the high concentrations require large amounts of the volatile anesthetic, and therefore -depending upon the fresh gas flow- entail high expenses.
From theoretical considerations of the pharmacokinetics of Desflurane an economic advantage is postulated in closed circle systems [3,6], but also in low flow anesthesia with fresh gas flows of 1 l/min and less an economical advantage should be possible [5].

Desflurane is a newer anaesthetic which may reduce recovery time, but because of its novelty, any potential benefits may be offset by its cost.
The purpose of this study is to determine whether desflurane- and isoflurane-based anaesthesia differ in cost, and if so, if there are any benefits associated with using one agent over the other.
Thus, the primary objective of this study is to determine the intraoperative and postoperative drug costs associated with desflurane anaesthesia compared to isoflurane anaesthesia in daycare patients undergoing arthroscopic menisectomy.

Researchers at The Children's Hospital used desflurane, an anesthetic gas, on piglets that underwent a low-flow type of cardiopulmonary bypass at one week of age.
Studies showed that the animals that received desflurane were better able to walk and feed, and had less injury to brain tissue, compared to piglets that received a combination of the liquid anesthetics fentanyl and droperidol.
Desflurane is a medication routinely used as an anesthetic, but now has been shown to also function as a neurological protector as well.

ASA: Desflurane at High Concentrations Appears to Have More Adverse Events Than Sevoflurane
Investigator Hariharan Shankar, MD, Pain Management Fellow, Department of Anesthesiology Medical College of Wisconsin in Milwaukee, Milwaukee, Wisconsin, said that the genesis of the study was a report last year that the two drugs had equivalent and minimal effects on airway irritation, at least at relatively low concentrations.
But after the mask cuff was re-inflated and the anesthetic concentration was increased to a MAC equivalent of 2, desflurane patients showed significantly more responses.
He concluded that desflurane irritates the airway enough so that its use could be a problem in some patients, especially at higher concentrations.

Before the induction of anesthesia, the patients of the desflurane and sevoflurane groups received fentanyl 2 microg/kg.
In all groups, anesthesia was induced with propofol and maintained either with remifentanil 0.25 microg x kg(-1) x min(-1), desflurane, or sevoflurane 0.85 MAC with 65% nitrous oxide in oxygen.
Thirty min after anesthesia administration, patients in the remifentanil-propofol and in the desflurane groups gave significantly more correct responses in the DSST compared with sevoflurane (remifentanil 87%, desflurane 83%, sevoflurane 56%), the impairment in the sevoflurane patients corresponding to the effects of a blood alcohol level of approximately 0.1% and, thus, being of clinical importance.
