Topic: Desmopressin

Desmopressin - Wikipedia, the free encyclopedia
Desmopressin (1-desamino-8-d-arginine vasopressin) is a modified form of the normal human hormone arginine vasopressin, an octapeptide (a chain of eight amino acids).
Desmopressin is used to reduce urine production in central diabetes insipidus patients and to promote the release of von Willebrand factor and factor VIII in patients with coagulation disorders such as type I von Willebrand disease, mild hemophilia A, and thrombocytopenia.
Desmopressin is not effective in the treatment of hemophilia B or severe hemophilia A. It may also be prescribed to reduce frequent bedwetting episodes in children by decreasing noctural urine production.

Desmopressin Acetate Injection Drug Information Desmopressin Acetate Injection
DESMOPRESSIN ACETATE has been shown to be more potent than arginine vasopressin in increasing plasma levels of factor VIII activity in patients with hemophilia and von Willebrand's disease Type I. Dose-response studies were performed in healthy persons, using doses of 0.1 to 0.4 µg/kg body weight, infused over a 10-minute period.
DESMOPRESSIN ACETATE is not indicated for the treatment of hemophilia A with factor VIII coagulant activity levels equal to or less than 5%, or for the treatment of hemophilia B, or in patients who have factor VIII antibodies.
DESMOPRESSIN ACETATE Injection 4 µg/mL should not be used in infants less than three months of age in the treatment of hemophilia A or von Willebrand's disease; safety and effectiveness in pediatric patients under 12 years of age with diabetes insipidus have not been established.

Desmopressin reduces the number of nights of primary noctural enuresis by at least 1 per week, and increases the likelihood of "cure" (defined as 14 consecutive dry nights) while treatment is continued (number needed to treat [NNT]=5-6) (strength of recommendation [SOR]: A, based on meta-analysis).
Desmopressin is an analogue of the natural pituitary hormone vasopressin acetate.
Desmopressin at doses of 20 [micro]g, 40 [micro]g, and 60 [micro]g similarly increased the likelihood of a cure (14 consecutive dry nights during treatment) in 3 trials reporting this outcome (relative risk for failure to achieve 14 dry nights with 20 [micro]g=0.84; NNT for cure=5.6).

The authors conclude that desmopressin administration is associated with a significant rise of ACTH and cortisol levels in majority of the patients with Cushing’s disease and there was good (non complete) concordance between the response to desmopressin test and surgical outcome.

Desmopressin is a drug that increases water reabsorption from the kidneys.
In one small trial, desmopressin resulted in more wet nights, compared to alarms, toward the end of treatment (weighted mean difference (WMD) 1.4; 95% CI 0.14 to 2.66), and the chance of failure or relapse after alarms was lower (RR 9.17; 95% CI 1.28 to 65.90).
Desmopressin rapidly reduced the number of wet nights per week experienced by children, but there was some evidence that this was not sustained after treatment stopped.
