Diagnosis-related group - Wikipedia, the free encyclopedia
Diagnosis-Related Group (DRG) is a system to classify hospital cases into one of approximately 500 groups, also refered to as DRGs, expected to have similar hospital resource use, developed for Medicare as part of the prospective payment system.
DRGs are assigned by a "grouper" program based on ICD diagnoses, procedures, age, sex, and the presence of complications or comorbidities.
DRGs have been used since 1983 to determine how much Medicare pays the hospital, since patients within each category are similar clinically and are expected to use the same level of hospital resources.

Diagnosis - Wikipedia, the free encyclopedia
Diagnosis (from the Greek words dia = by and gnosis = knowledge) is the process of identifying a disease by its signs, symptoms and results of various diagnostic procedures.
The history of medical diagnosis began in earnest from the enlightened days of Hippocrates in ancient Greece but is far from perfect despite the enormous bounty of information made available by medical research including the sequencing of the human genome.
The practice of diagnosis continues to be dominated by theories set down in the early 1900s.

We studied the diagnosis-related groups (DRG) impact of nutrition support on 80 consecutive cardiac surgery patients operated upon during a 6-month period.
Patients were arbitrarily placed into three outcome groups: group I consisted of seven patients who received postoperative nutrition support; group II included 38 patients who received no nutrition support and did not develop complications; Group III consisted of 35 patients who received no nutrition support but developed postoperative complications.
Group I patients were significantly older than groups II and III (p less than 0.0003) and had a significantly longer average length of stay (ALOS) (p less than 0.001), ALOS in SICU (p less than 0.0001) and greater incidence of both septic complications (p less than 0.02) and mortality (p less than 0.02).

The formula used to calculate payment for a specific case takes an individual hospital's payment rate per case and multiplies it by the weight of the DRG to which the case is assigned.
Each DRG weight represents the average resources required to care for cases in that particular DRG relative to the average resources used to treat cases in all DRGs.
Cases are classified into DRGs for payment under the prospective payment system based on the principal diagnosis, up to eight additional diagnoses, and up to six procedures performed during the stay, as well as age, sex, and discharge status of the patient.

 CFIDS
Diagnosis of chronic fatigue and immune dysfunction syndrome (CFIDS), also known as chronic fatigue syndrome (CFS), is a time-consuming and difficult process which is generally arrived at by excluding other illnesses with similar symptoms and comparing a patient's symptoms with the 1994 International case definition.
Many diseases have similiar symptoms including lupus, hypothyroidism and Lyme disease and these need to be considered when making a diagnosis.
In 1994 an updated case definition was developed by an international working group of CFIDS experts.

Hospital employment under revised medicare payment schedules - Diagnosis Related Groups-based medicare payments' ...
Payment to hospitals for inpatient care is assigned according to four criteria: a patient's principal diagnosis or surgical procedure, whether there is an important secondary diagnosis, the patient's age, and whether or not the patient was alive upon discharge.
The DRG schedule is applied to inpatient services rendered to medicare beneficiaries.
Uncertain about the eventual effects of DRG's on revenues and anticipating a need to reduce costs across the board, hospitals with large medicare caseloads attempted to reduce admissions of less acutely ill patients and shorten hospital stays before the new system was implemented.

The assignment of peer groups allows a fairer comparison of hospital performance within the state; assignment was initially performed in 1994.
For example, if a hospital’s discharges had an average severity of 1.5, its LOS would be compared to the peer group's value halfway between the peer group's LOS for a severity of one ("1") and its LOS for a severity of two ("2").
For example, if a hospital’s discharges had an average severity of 1.5, its Total Charges would be compared to the peer group's value halfway between the peer group's Total Charges for a severity of one ("1") and the peer group's Total Charges for a severity of two ("2").

Specifically, by educating physicians on how the DRG coding system works for hospitals and linking the value of improving inpatient documentation directly to the physician’s bottom line, hospitals now have motivated physicians to improve inpatient documentation.
The point of commonality for both DRG coding for hospitals and EandM coding for physicians is the documentation of multiple medical diagnoses.
After viewing this program, the linkages are clear and the education allows the audience the ability, with a copy of the Pocket Guide to Clinical Coding, to immediately transform their documentation to support a compliance correct, Medical Decision Making driven, accurate EandM CPT code.
