M.E.Müller: Atlas: Segment 42   (Site not responding. Last check: 2007-10-31)
The diaphyseal fractures of the tibia (10,84% of the total) have historically been the center of the discussion about conservative versus surgical treatment and, regarding internal fixation, there still is discussion between the advocates of the intramedullary nailing procedures and the plate proponents.
The tibial diaphysis is the testing ground for the majority of the surgeons; However, it was never so for us because it was precisely with the tibia that we learned how to apply the procedures of the AO technique.
The tibial diaphysis is a quite accesible bone on which meticulous approaches can be carried out, allowing a very precise performance of the previously planned compressed internal fixation.

The diaphysis is largely complete with some fill in missing areas, the sternal end is broken off irregularly through the distal costo-clavicular facet, and the distal end is preserved to the middle of the acromial end with bone loss dorsally and on the conoid tubercle tip.
Largely intact diaphysis with minor root etching, the trabecular base of the postero-medial medial condyle, the distal end of the anterior tibial tuberosity, and the talar trochlear surface with portions of the anterior and posterior articular margins.

THE MERCK MANUAL OF GERIATRICS, Ch. 22, Fractures
A typical long bone is divided into the diaphysis (shaft), the epiphysis (the articular region at each end), and the metaphysis (the flared region joining the diaphysis and epiphysis).
The diaphysis is composed of cortical bone, whereas the epiphyses and metaphyses are composed primarily of trabecular bone.
With age, the diameter of the diaphysis increases as bone is resorbed from the inner (endosteal) surface and is added to the outer (periosteal) surface.

eMedicine - Osteofibrous Dysplasia : Article Excerpt by: Robert Mervyn Letts, MD, FRCSC, FACSC
Most lesions of osteofibrous dysplasia affect the cortex of the tibia, predominantly the middle third of the diaphysis.
They further reported that 22 of 35 lesions (63%) affected the middle third of the tibial diaphysis.
However, in a study of 5 children reported by Ozaki et al, one child presented with bilateral lesions of both ulnae and tibiae.

Like the femur, the tibia is described as having a proximal end or head, the middle portion or shaft (diaphysis), and a distal portion.

Lamellae which run parallel to the surface of the bone are visible both on the outer, convex surface of the bone (circumferential lamellae) and on the inner, concave surface of the bone facing the marrow cavity (endosteal lamellae).
the manchette of bone forming around the diaphysis of the cartilage model of the bone, is formed by a mechanism similar to intramembranous ossification and results in the deposition of woven bone.
Between the diaphysis and the epiphyses a thin sheet of cartilage, the epiphyseal plate, is maintained until adulthood.

Skeletal System / Bone Development and Growth
After spongy bone is formed in the diaphysis, osteoclasts break down the newly formed bone to open up the medullary cavity.
Ossification in the epiphyses is similar to that in the diaphysis except that the spongy bone is retained instead of being broken down to form a medullary cavity.

The articulating surfaces of the humeral condyles are called the trochlea, which apposes the ulna, and the capitulum, which apposes the radial head.
The variations that exist are minor: the diaphysis is of a more uniform diameter than in the dog, diaphyseal curvature is less, the supratrochlear fossa is not completely penetrated, and finally the medial epicondyle possesses an epicondyloid fossa through which the ulnar nerve passes (Fig.
