A Document Type Definition (DTD for short) is a set of declarations that conform to a particular markup syntax and that describe a class, or "type", of SGML or XML documents, in terms of constraints on the structure of those documents.
A DTD may also declare default attribute values, named entities and their replacement text, and other constructs that are not always required to establish document structure, but that may affect the interpretation of some documents.
A DTD is associated with a particular document via a Document Type Declaration, which is a bit of markup that appears near the start of the associated document.

The DSD solution to the problem is not as general, but often workable: it stipulates that variations of a grammar occur in different contexts (as understood by the position in the document explained by the path from the root).
DSDs represent very much a grammar approach as well, but the need to invent names for various uses of the same element name can sometimes be reduced drastically by instead characterizing their occurrences through context expressions.
DSDs use Boolean logic to connect attribute occurrences and content for greater expressive power, but the use of regular expressions also for attributes in RELAX NG simplifies the syntax.

Of course, it depends on the complexity of the source documents, and the level of markup that is required in the target documents.
But there is nothing in the PDF document that specifies that certain text is in column one and certain text is in column two (or even that there are in fact two columns on the page).
