Dolasetron and its active metabolite, hydrodolasetron (MDL 74156), are selective 5-HT receptor antagonists shown not to have activity at other known serotonin receptors and with low affinity for dopamine receptors.
However, dolasetron is not recommended in patients with severe renal impairment because of the possibility of prolonged QTc intervals and other cardiac conduction abnormalities from elevated hydrodolasetron levels.
Dolasetron did not inhibit the antitumor activity of 4 chemotherapeutic agents (cisplatin, 5-fluorouracil, doxorubicin, cyclophosphamide) in 4 murine models.

As the results of the study found that the two largest doses of dolasetron and the dose of ondansetron were more effective than the smallest dose of dolasetron, the authors argued that this supported the internal validity of their conclusions.

As dolasetron was the last of the three agents in clinical development, most of the trials involving dolasetron included specific ECG monitoring, resulting in more detailed ECG data for this compound.
Specifically, dolasetron caused acute effects on the ECG via sodium channel blockade to delay ventricular depolarization, manifested as prolonged PR and QRS duration, without a substantial effect on ventricular repolarization (QT interval).
Dolasetron, ondansetron, and granisetron also have, as a class, some effects on ECG intervals.

That was the principal conclusion of the first multicentred trial to asses the comparative antiemetic efficacy and safety of dolasetron mesylate, a new type three 5-hydroxytryptamine (5-HT3) receptor antagonist, and ondansetron in the prevention of chemotherapy-induced emesis.
Doses of 1.8 mg/kg dolasetron, 2.4 mg/kg dolasetron, or 32 mg ondansetron resulted in complete response rates of 49.2%, 45.6% and 50.4%, respectively, in the low-dose cisplatin group, and 36.8%, 31.3%, and 31.8%, respectively, in the high-dose cisplatin group.
