Donepezil is used to treat dementia (a brain disorder that affects the ability to remember, think clearly, communicate, and perform daily activities and may cause changes in mood and personality) associated with Alzheimer's disease (AD; a brain disease that slowly destroys the memory and the ability to think, learn, communicate and handle daily activities).
Donepezil may improve the ability to think and remember or slow the loss of these abilities in people who have AD.

British Journal of Pharmacology - Donepezil modulates nicotinic receptors of substantia nigra dopaminergic neurones
The effects of donepezil, one of the most common cholinesterase inhibitors used for treatment of Alzheimer's disease, were studied on nicotinic receptors (nAChRs)-mediated postsynaptic currents, in dopaminergic neurons of the substantia nigra pars compacta, using the patch-clamp recording technique in slice preparations.
The deactivation of the current evoked by brief puffer pulses of nicotine (50 ms) was left unchanged by donepezil application, indicating that the reduction in the peak amplitude was not due to a change in the kinetics of the channel.
Donepezil, a tertiary amine which is almost fully protonated at physiological pH, may block receptor channels opened by nicotine in a manner similar to that of other substances, like local and general anaesthetics (Neher and Steinbach, 1978; Mori et al.

In a press statement, Dr. Mohs also said that the findings of this study suggest that prolonged treatment with donepezil is an important weapon to help patients improve or maintain their independence while living with the disease.
The study shows that about 56 percent of the placebo group compared with 41 percent of the donepezil group had criteria for clinically evident functional decline and were removed from the study.
The risk of clinically evident functional decline of the patients receiving donepezil was practically 62 percent of that of the placebo group.

Alzheimer's prevention trial early results, July 16, 2004 Press Release - National Institutes of Health (NIH)
The reduced risk of progressing from MCI to a diagnosis of AD among participants on donepezil disappeared after 18 months, and by the end of the study, the probability of progressing to AD was the same in the two groups.
Donepezil was selected because of its current approval as a drug for treating patients already diagnosed with AD.
These secondary analyses suggest that decline among the group on donepezil occurred at a slower rate on tests of global cognition, memory, and language than the other participants during the first half of the study but progressed at the same rate thereafter.

Clinical Trial: Donepezil to Treat Dementia in Parkinson's Disease
Donepezil is approved for treating patients with Alzheimer's disease, whose memory and cognition problems are similar to those of patients with Parkinson's disease who are affected by dementia.
Donepezil prevents the breakdown of a chemical messenger called acetylcholine, which is involved in memory and other cognitive functions, and may improve cognition in patients.
The proposed study will determine whether a therapeutic intervention with donepezil, an acetylcholinesterase inhibitor, shown to be effective in AD, will improve cognitive function in PD. The study is planned as a 26-week, randomized, double-blind, placebo-controlled, cross-over study of the efficacy and safety of donepezil in PD with dementia.
