Dothiepin (UK PID)   (Site not responding. Last check: 2007-08-17)
Dothiepin is metabolised by demethylation and S-oxidation in the liver, resulting in the active metabolites, nordothiepin (also known as northiaden or desmethyldothiepin), dothiepin sulphoxide and nordothiepin sulphoxide, all of which contribute to the antidepressant effect (Rees 1981).
Concentrations of dothiepin, nordothiepin, dothiepin-S-oxide and nordothiepin-S-oxide were measured in blood and milk samples from five breast feeding women, and in plasma samples from their infants.
Dothiepin overdose should be managed on a clinical basis rather than on the amount ingested, but as a guide, doses of 1 g in adults have been associated with severe toxicity.

Dothiepin is readily absorbed from the gastro-intestinal tract, and extensively demethylated by first pass metabolism in the liver to its primary active metabolite, northiaden.
Dothiepin is excreted in the urine, mainly in the form of metabolites; small amounts are also excreted in the faeces.
Dothiepin has an adverse effect on the myocardium and can cause conduction defects and cardiac arrhythmias; an increased risk of sudden death has been suspected in cardiac patients receiving tricyclic antidepressants.

 Antidepressants
The following antide-pressants were included in the study: the tricyclic antidepressants amitriptyline, dothiepin, and lofepramine and the atypical antidepressant bupropion, which all have considerable market shares in the UK and/or US markets; the selective serotonin reuptake inhibitors (SSRIs) citalopram, fluoxetine, fluvoxamine, paroxetine, and sertraline; and the recently approved antidepressants venlafaxine and nefazodone.
Dothiepin has equipotent 5-HT and NA reuptake inhibitory activity, whereas northiaden shows a slight selectivity for NA reuptake inhibition.
The slow elimination rate of northiaden (36-46 hr) compared to dothiepin (14-24 hr) suggests that northiaden contributes significantly to the therapeutic effect of dothiepin.

To determine whether concerns surrounding SSRIs are warranted, the team estimated the relative risks of nonfatal suicidal behavior associated with four antidepressant medications — the SSRIs fluoxetine and paroxetine, and the tricyclics amitriptyline and dothiepin — using the UK General Practice Research Database for 1993 to 1999.
In all, 159,810 patients starting treatment with one of these four antidepressants were identified, with data for 555 patients who showed suicidal behavior and 2062 individuals who did not used for the current study.
The results, published in the Journal of the American Medical Association, showed that the relative risks for newly diagnosed nonfatal suicidal behavior were similar for the four antidepressants, at 0.83 for amitriptyline, 1.16 for fluoxetine, and 1.29 for paroxetine compared with the reference drug dothiepin.

Initial days of antidepressant therapy associated with greatest risk of suicidal behavior - Formulary
The study focused on the 4 most widely used antidepressants in the United Kingdom—the selective serotonin reuptake inhibitors (SSRIs) fluoxetine (n=176) and paroxetine (n=135), and the tricyclic antidepressants amitriptyline (n=77) and dothiepin (n=167), with dothiepin serving as a reference group.
The adjusted odds ratio estimates comparing dothiepin with amitriptyline, fluoxetine, and paroxetine for nonfatal suicidal behavior were 0.83 (95% CI, 0.61-1.13), 1.16 (95% CI, 0.90-1.50), and 1.29 (95% CI, 0.97-1.70), respectively.
The OR comparing dothiepin and paroxetine approached statistical significance; however, according to the study authors, it was low enough that it could have been attributed to uncontrolled confounding by severity of depression.
