There have been rumors that in those later episodes at the end of the series, somewhere buried in all of those Capeman stories, Dr. Claw's face is revealed to Gadget and the viewers.
Produced in 1992, the Dr. Claw figure came from a line of Gadget figures produced by DIC Enterprises (the company that created and produced the cartoon) and Grand Toys.
Amongst multiple Gadget figures in different colored trench coats and a Penny and Brain 2-pack was Dr. Claw himself, his face shrouded in cardboard so that you'd have to buy him to see his face.

Claw's face was ultimately revealed in non-canon sources such as licensed action figures and a Nintendo video game, which depicted him as an aging scientist with bushy whitish hair and a permanent scowl on his face.
Claw's voice is actually the unmodulated voice of Soundwave from the Transformers; this was revealed in a few episodes of Transformers when mistakes were made in the voice editing.
Claw then demanded what the cat was laughing about, adding furiously, "I did all the work!" An episode or two later, Dr. Claw was engaged in tongue-lashing his minions.

The "Dr." on his name is purely a title, because attaching something like "Mr." or "Dr." or "Mrs." or "Esquire" to a vicious sounding name makes it flashier, which is a general rule of thumb among villains.
He chose "Dr. Claw" as his new moniker simply because he was drunk and he thought it sounded funny.

Piratebowling: If i recall correctly, there was indeed an episode where Dr Claw stood up and turned around, but it turned out to be a double, or a clone, or a robot, or a lackey, or somesuch.
I think with his steel spiked claw and the MAD logo, there was a certain ubiquitous template of metallic skull villain faces that kids watching that show would automatically roughly imagine taken from all their other cartoons and comics.
