 145 Wn.2d 65, MILLER v. JACOBY
Whether the drain was negligently placed during surgery is not readily observable to laypersons, and thus expert testimony must be presented to establish the standard of care necessary during the procedure.
whether expert medical testimony is necessary to a determination that health care providers' actions, in placing a Penrose drain in a patient's body during surgery and in not removing the entire drain postoperatively, constitute negligence.
Nevertheless, as Judge Appelwick notes in his dissent, "The moment of Dr. Jacoby's incomplete removal of the drain resulted in a foreign object that was placed in the body intentionally and temporarily during surgery, then becoming a foreign object inadvertently and permanently left in the patient at the completion of surgery."  Med-Lib - Medical Online Library - English Articles - Oxford Textbook of Surgery - Principles and practice of thoracic drainage
The chest drain should never be clamped in the presence of an air leak because of the risk of tension pneumothorax.
The intercostal drain is simply a conduit used to remove air, blood, pus, or fluid from the pleural cavity, thereby preventing compression of the lung and allowing it to re-expand.
The extent of a pneumothorax is determined on the chest radiograph by measuring the distance from the lung border to the inside of the thoracic wall at the level of the anterior bony end of the third rib.

Three days after the surgery, appellee Dr. Richard Nix, another orthopedic surgeon at the Clinic, was making rounds and pulled the penrose drain tube out of Mary's leg.
They contend that Dr. Schultz's deposition, in which he averred that the piece of penrose drain left in Mary's leg probably caused her infection and should have been removed, presented a fact question that was sufficient to survive a motion for summary judgment.
A penrose drain tube was placed in her leg to drain any infectious materials from the wound.

Mulitfunction chest drainage systems (Redax) have been designed primarily for post-cardiothoracic surgery and chest trauma management.
The basic requirements are a suitable chest drain with minimal resistance, an underwater seal and a collection chamber.
Insert the drain into the pleural cavity and slide into position (usually towards the apex).

 Optimal Design of Small
The need for chest drainage is also required following open heart surgery and chest trauma to evacuate any pooling blood which, if left in the mediastinal cavity, can cause cardiac distress or tamponade.
There are two basic considerations relative to the design of a chest drain from a functional point of view and patient comfort; size and flexibility.
24F chest drain with the length of 20cm was used in this calculation and the radius of the hole size was 0.2cm.

Mediastinal chest drain clearance for cardiac surgery (Cochrane Review)
Mediastinal chest drains (including pericardial drains) are inserted as standard post-operative practice following cardiac surgery to assist the clearance of blood from the pericardial space and to prevent cardiac tamponade.
Authors' conclusions: There are insufficient studies which compare differing methods of chest drain clearance to support or refute the relative efficacy of the various methods in preventing cardiac tamponade.
Nor can the need to manipulate chest drains be supported or refuted by results from RCT's.

Atrium produces a broad spectrum of medical device technologies used for open heart surgery, emergency chest trauma, thoracic drainage and surgical repair of diseased blood vessels.
Atrium recently launched several new chest drainage products, including the world’s first truly "wearable chest drain" to allow patients the benefits of early ambulation with the latest in mobile chest drain technology.
A unanimous jury had found in the November 2002 decision that Atrium's Express Dry Suction Chest Drains did not infringe the five chest drain medical device patents that were litigated by Genzyme.

 Damage Control Surgery
One end of the Penrose drain is sutured closed and the red rubber catheter is placed in the drain.
A similar technique, involving a balloon tamponade device consisting of a Penrose drain and a red rubber catheter, is used to tamponade bilobar injuries.
Because there is often considerable fluid loss from the abdominal wound before the definitive closure, the surgeon may position closed suction drains under an adhesive drape to allow for fluid collection and protection of the surrounding skin.

Drain in incision? - Hyster Sisters Hysterectomy Archive
In any case.....The only reason I had a drain, is because my bladder was nicked...and besides leaving a catheter in for a week (to keep traction off my bladder, so it can heal), the urologist also recommended the drain, I guess maybe to prevent fluid buildup from putting pressure on the outside of the bladder.
I do remember having a drain many years ago when I had a lump removed from my breast and yes, it was very painful.
I doubt if you'll be having a drain, but you might want to discuss it with your doctor just to be sure.

PURPOSE: Prospective studies in the general surgery literature have shown fewer wound related complications with closed suction drainage than with open passive drainage.
Drain type was compared with duration of use and the incidence of relevant complications.
RESULTS: A Penrose drain was used in 37.6% (74 of 197) of partial nephrectomies and a closed suction drain was used in 62.4% (123).

‘Bloodless’ liver transplant for boy from Lahore
‘‘Safi’s haemoglobin count was 10.5 after the surgery so we had to drain some blood for the haemoglobin to go down to the required 8.5,’’ said Dr Neelam Mohan, paediatric hepatologist at the hospital.
The ‘‘bloodless;’ surgery, at Sir Ganga Ram Hospital, was the first procedure of its kind to be performed in the country.
According to doctors, bloodless surgery translates into less infection, less ventilator requirement, fast post-transplant recovery, no transfusion-related reactions and higher success rates.

allnurses.com Nursing for Nurses - RN's removing surgical drains
What is new is that we have some surgeons are placing JP drains or even "mini" chest tubes in the pleural space following open heart surgery.
11-04-1999, 08:26 AM I have removed penrose drains, and 1 JP, but if I were requested to do it again I might want to review a procedure book, it has been about 7 years.
However, we also have to show the tip (mostly bellovacs) to another RN and she must co-sign in the patients chart that the tip was sighted.

Deadly sutures in a hospital near you?
I spoke to a gentleman who four days after heart surgery had an abscess the size of a basketball on his chest where the surgery was, so they actually had to drain it surgically following his surgery.
They take the sutures and repackage them in their own surgery kits, so on the hospital shelves, you would not see just vicryl sutures; you'd see "Baxter's Surgery Kit." So these sales reps are only looking for the vicryl sutures in the Ethicon packaging and not Baxter's Surgery kits or any other distributors' surgery kits.
And the way they handled the recall -- it was done in such a quiet, secretive way that even the doctors who were doing the surgery and using these sutures day in and day out did not know that they were contaminated.

 Craniotomy
After the operation is finished, the piece of excised bone is replaced, the muscle and skin are stitched up and a drain is placed inside the brain to remove any excess blood left from the surgery.
Recovery depends on many factors, including the type and severity of brain injury, the type of surgery and whether or not there were neurological deficits before surgery.
There are many different types of brain surgery, but the recovery process following craniotomy is much the same in most cases.

 Ganglions
Cysts on the back of the end joint of the finger (mucus cysts) may gradually thin and weaken the overlying skin, allowing liquid to drain out but also allowing bacteria to enter the joint, sometimes causing a serious infection.
Cysts on the front of the wrist (volar wrist ganglions) are more likely to come back after surgery than cysts on the back of the wrist (dorsal wrist ganglions).
Cysts coming from joints are less likely to come back after surgery if the joint itself is "cleaned out", but cleaning the joint out increases the chance that the joint will be somewhat stiff after surgery.

 Laryngeal Cancer
Primary subglottic cancers, which are quite rare, drain through the cricothyroid and cricotracheal membranes to the pretracheal, paratracheal, and inferior jugular nodes, and occasionally to mediastinal nodes.
Salvage is possible for failures of surgery alone or of radiation therapy alone and further surgery [5] and/or radiation therapy should be attempted, as indicated.
Advanced laryngeal cancers are often treated by combining radiation and surgery.
