  Duloxetine (Cymbalta®)
Duloxetine hydrochloride is a selective serotonin and norepinephrine reuptake inhibitor (SSNRI) that is indicated for the treatment of major depressive disorder (MDD) and for the management of neuropathic pain associated with diabetic peripheral neuropathy (DPN).
Duloxetine was approved in August 2004 for treating MDD and is considered an alternative to other antidepressants to treat the emotional and painful physical symptoms of depression.
Duloxetine and thioridazine should not be co-administered because of the increased risk of ventricular arrhythmias and sudden death associated with elevated plasma levels of thioridazine.

Sept. 10, 2004 — Duloxetine, a serotonin and norepinephrine reuptake inhibitor, is effective for the treatment of fibromyalgia with or without major depressive disorder, according to the results of a randomized trial published in the September issue of Arthritis and Rheumatism.
Independent of baseline status of major depressive disorder, duloxetine treatment improved fibromyalgia symptoms and pain severity, and the treatment effect on significant pain reduction in women was independent of the effect on mood or anxiety.

Clinical studies of duloxetine for the treatment of SUI have shown the most commonly reported adverse events (incidence of > 5 percent and at least twice the placebo rate) reported by patients receiving duloxetine have been nausea, dry mouth, fatigue, insomnia, constipation, headache, dizziness, somnolence (drowsiness) and diarrhea.7
Duloxetine is a balanced dual reuptake inhibitor of the neurotransmitters serotonin and norepinephrine,8 which play a key role in normal urethral sphincter closure, the weakening of which can result in SUI.
By increasing neurotransmitter concentration, duloxetine is believed to increase the tone and contraction of the external urethral sphincter,9 which helps prevent accidental urine leakage during physical activities such as sneezing, coughing, laughing, lifting or exercise.

The drug manufacturer, Eli Lilly and Co, has been ordered not to accept new participants in the US trial of the compound duloxetine after the 19-year-old was found hanged in a room at a company research laboratory in Indianapolis this week.
She was one of 25 volunteers in the trial of the drug compound, which the company hopes to use as the main ingredient in an anti-depressant, and as a treatment for stress urinary incontinence.
Professor Moore, who was involved in clinical trials of duloxetine at Sydney's Prince of Wales and St George hospitals, said the compound was revolutionary and offered the first effective drug treatment for stress incontinence - estimated to affect at least 78 million women over the age of 20 worldwide.
