  Drucker on Effectiveness
I've had a lot to say on the difference between agile development methods and their predecessors is a focus on effectiveness rather than cost accounting style efficiency.
He goes on to observe that effectiveness is not achieved from principles but from successful execution of practices.
"Effectiveness is a habit; that is, a complex of practices.

Protective Effectiveness of Hantavirus Vaccine | CDC EID
Because the effectiveness was calculated by comparing each dose (exactly one to three) with no vaccination, case-patients or controls not applicable to that comparison were excluded from each matched set.
Estimates of vaccine effectiveness according to the number of doses received rose from 25% (95% CI –78% to 68%) for one dose to 46% (95% CI –35% to 78%) for two doses to 75 % (95% CI –18% to 95%) for three doses.
In particular, effectiveness increased when persons for whom >1 year had passed since their second dose were excluded, which suggests that the protective effect of the second primary vaccination does not persist beyond the period recommended for having the booster dose.

 Foundation Effectiveness: Definitions and Challenges
These interviews probed foundation leaders' understanding of effectiveness, the methods they use to judge it, and how they say their foundations have changed or need to change in order to be more effective.
Often the major challenge to achieving effectiveness is that: a) effectiveness goals are overshadowed by other priorities, or b) certain dimensions of effectiveness being pursued by the foundation are not compatible with its other effectiveness goals.
The foundation effectiveness framework is not applicable to some foundations that conceive of themselves not as organizations at all, but as instruments or extensions of individual philanthropy.

Cost-Effectiveness of Strategies To Enhance Mammography Use
Effectiveness data for our analysis were based on a randomized trial (8) that evaluated alternative strategies to improve participation in GHC's breast cancer screening program.
Marginal cost-effectiveness was calculated by dividing the change in cost by the change in effectiveness and was determined for the postcard relative to the initial letter recommending mammography, for the reminder call relative to the postcard, and for the motivational call relative to the reminder call.
Although the reminder call was more effective than the postcard, our analysis showed that the reminder call's greater unit cost outweighed its greater relative effectiveness; therefore, the postcard was more cost-effective as a promotional strategy.

 Contraceptive Method Effectiveness
The effectiveness rates of these methods are expressed in terms of how many women experience pregnancies during their first year of using the method.
It is very important to note that the effectiveness of LAM is highly dependent on the woman exercising full or nearly full breastfeeding (the baby nurses every three to four hours and takes little or no supplements) during the baby's first six months only if the mother has not resumed her period.
The effectiveness of using an IUD or ECPs for emergency contraception is dependent on the time in a woman's cycle she had intercourse, therefore making it difficult to compare data from studies.

Nevertheless, the decline in effectiveness from the fleet tests to the evaluations was clear-cut, even taking into account that the data bases were not perfectly comparable (participant-reported vs. police-reported crash data).
CHMSL effectiveness in the struck vehicle in a front-to-rear collision is about the same whether the driver of the striking vehicle is young or old, male or female.
Although the observed point estimate of effectiveness for CHMSL in light trucks (5.0 percent) is close to the lamps' long-term effectiveness in passenger cars (4.3 percent), the uncertainty in the light-truck estimate, at this time, does not yet permit the inference that the lamps are equally effective in cars and trucks.

Preliminary Assessment of the Effectiveness of the 2003--04 Inactivated Influenza Vaccine --- Colorado, December 2003
However, additional studies are needed to evaluate the effectiveness of the 2003--04 vaccine against laboratory-confirmed influenza and influenza-related complications, including hospitalization and death.
Two methods were used to estimate vaccine effectiveness to account for changes in vaccination status during the influenza outbreak period: a categorical analysis and a person-time analysis.
Point estimates for vaccine effectiveness were 14% and 3%, respectively, with 95% confidence intervals for both estimates that included zero (Table 2).

However, identifying the most effective type of treatment and for whom it is most effective continues to be a difficult task.
The clinical measures of effectiveness are particularly difficult to discern due to the enormous diversity within the modality and the relative scarcity of available outcome studies.
When discussing treatment protocols and effectiveness, it is helpful to know the number of people in the United States currently involved in treatment for substance abuse and to understand the characteristics of the population in treatment.

Effectiveness alone, however, is not a sufficient basis to initiate services in most practical health care contexts.
Factors other than effectiveness, reflecting the immediate trade-offs and broad implications of providing a service, are relevant to the goals and the practical constraints confronted by every decision maker.
An effective intervention may be clinically inconsequential, or it may help so few of the individuals to whom it is offered that it is not worth implementing.

 Evaluating Effectiveness
External effectiveness, on the other hand, addresses issues related to your philanthropy that exist outside your family or philanthropic vehicle, such as grantee relationships or how to measure tangible impact.
When evaluating the effectiveness of your philanthropic vehicle, large or small, you first may want to consider how clearly stated and understood your goals are among the family and/or staff members involved.

Beyond these general points, the higher costs of establishing effectiveness in a trial—largely driven by statistical power, but also by considerations such as the difficulty of identifying and enrolling patients suitable for the trial and the burdens placed on patients who decide to enroll—tend to make an evidence hurdle higher.
Effectiveness appears to trump cost as an influence on coverage decisions: According to medical directors, 93 percent of all plans and 98 percent of large plans will cover a more effective intervention, even if it is more costly.
Procedures that are effective but not cost-effective in any identifiable patient population might be subject to high copayments or fixed, high coinsurance rates (percentage payments rather than fixed-dollar amounts), with no individual adjustments.

It is recommended that you reserve the right to orally request acceleration of the effective date pursuant to Rule 461.
In order to request effectiveness orally, you should include a letter in your initial filing package on behalf of issuer and, if the offering is underwritten, the managing underwriter, indicating both parties' intention to request acceleration orally and an acknowledgment of their obligations under the 1933 Act.

The cost-effectiveness analysis points to the need for data on patient utilities associated with life after orchiectomy; re-evaluation of the risks of lower doses of DES as method of androgen suppression; and collection of economic data from randomized controlled trials as additional priorities for future research.
Combined androgen blockade has not been demonstrated to be of greater benefit than monotherapy for the aggregate population of patients with advanced prostate cancer or for the subpopulation of patients with good prognostic factors.
Randomized controlled trials are needed to assess the effectiveness of various strategies for the timing of androgen suppression, and should be prospectively designed to address whether there are subgroups of patients more likely to benefit from early initiation of androgen suppression.

 Primer on Cost-Effectiveness Analysis
The final outcome measure for the analysis is the CE ratio: the ratio of marginal cost to marginal effectiveness.
Even if the effectiveness data are from randomized trials, it's important to ask whether they really pertain to the population and setting in which the strategy is likely to be applied.
Analyses with CE ratios of $50,000 per quality-adjusted life-year may conclude with an assertion that the analyzed strategy is "cost-effective." Whether or not this helps anyone make a decision is hard to know.

NIDA - Measuring and Improving Costs, Cost-Effectiveness, and Cost-Benefit for Substance Abuse Treatment Programs - ...
Measures for cost, treatment procedures, effectiveness, and benefits are developed and tested over several weeks.
Two weeks are allotted for development and testing of effectiveness measures because these measures are crucial to the rest of the process, because most staff want to contribute to this part of the analysis, and because there are so many measures of effectiveness.
If the monetary value of your program's outcomes is relatively easy to derive from information on program effectiveness, or if direct measures of benefits are easy to come by, you may want to make the extra effort to do a cost-benefit analysis in addition to your cost-effectiveness analysis.
