He gives the time he was stranded on 20th century Earth for 30 years (Future's End, Part I) and another, a "temporal inversion in the Dekara Sector." I would guess that that would refer to Janeway's destruction of the Krenim ship in Year of Hell, Part II, or maybe the events of Time and Again.
He never names to the third, but I would guess that it is Admiral Janeway's interference in Endgame.
I'd figure that the third one would have to be Endgame- Janeway comes from the future, and doing so changes a fair amount of history (Voyager gets home earlier, the Borg are dealt a crippling blow, etc).

The puzzles are a large part of it; those in the endgame are particularly good, I think, and the microscope problem was rewarding to figure out.

Perhaps the most welcome thing about the endgame was that the goal was clear and the territory to explore limited; there was no question of wandering around looking for the right object only to find that the solution actually turned on a bad pun.

The prayer system is particularly elegant, almost serving as get-out-jail-free cards - I think in every case, the player can find a solution which doesn't involve prayer, but if you're having trouble coming up with the answer, a saint's intercession will do the job, without forcing recourse to the hints file.
Not only is the narrative twist nicely done - it both comes out of nowhere and had me slapping my forehead for not noticing it sooner - there's also a mechanical twist, as this whole time the game has been keeping track of the sins you've committed.
My notes don't record any disambiguation issues or typos; they're basically just reminders not to forget how neat particular elements were.
