Epstein-Barr virus - Wikipedia, the free encyclopedia
The Epstein-Barr virus (EBV), also called Human herpesvirus 4 (HHV-4), is a virus of the herpes family (which includes Herpes simplex virus and Cytomegalovirus), and is one of the most common viruses in humans.
Epstein-Barr was the first virus to be identified as an oncovirus, meaning that it is associated with the development of cancer.
Epstein-Barr virus primarily infects B cells, where it forms a latent infection that persists for life.

EBV hepatitis usually also shows portal and periportal mononuclear infiltrates of varying severity, comprised of small and blastic lymphocytes, some of which are atypical, admixed with plasmacytoid lymphocytes and plasma cells(1-6).
The diagnosis of EBV hepatitis or PTLD is generally suspected on the routine H&E slides and then confirmed by in situ hybridization for EBV RNA (EBER sequence).
EBV hepatitis typically shows features of a "reactive" or low-grade hepatitis with focal hepatocellular swelling, mild acidophilic necrosis of hepatocytes and mild lobular disarray(1-6).

Epstein Barr Virus in the Pathogenesis of NPC
The association of Epstein-Barr virus (EBV) with a number of cancers, and the ability of EBV to latently infect and immortalize B cells in vitro, has led to an intense interest in identifying the mechanisms by which EBV is able to transform B cells.
EBV is a B lymphotrophic virus whose biology is closely entwined with the normal biology of B lymphocytes.

EBV was at one time believed to be the cause of the chronic fatigue syndrome, but chronic infection with this virus actually produces a separate (if similar) disorder.

 AHMF: References for Epstein-Barr virus
Epstein-Barr virus infection may not be self-limiting, and the virus may be associated with clinically recognizable illness other than infectious mononucleosis in children as well as in adults.
The relationship of this fatigue syndrome to EBV is unclear; further studies are needed to determine its etiology.
These findings suggest that antibodies against EBV DNAP may be a useful marker in delineating a subset of patients with severe fatiguing illness for appropriate treatment trials and for monitoring their outcomes.

EBV is the causative agent of infectious mononucleosis and has been linked to the development of several malignant tumours, including B-cell neoplasms such as Burkitt’s lymphoma and Hodgkin’s disease, certain forms of T-cell lymphoma, and some epithelial tumours, such as undifferentiated nasopharyngeal carcinoma and a proportion of gastric cancers.

Epstein-Barr virus appears to cause at least two malignancies common in specific locations: Burkitt's lymphoma, a B cell cancer frequently found in Africa, and nasopharyngeal carcinoma, a tumor in the back of the throat that's the most common cancer among men in many parts of Asia.
David Thorley-Lawson of Tufts University School of Medicine in Boston focuses on how Epstein-Barr virus persists in the body rather than how it infects a person initially.
Later, virus growing in B cells is shed into the infected person's saliva, where it's ready to start the cycle again in someone else's epithelial cells.

The most commonly used method of diagnosing EBV is by monitoring for the presence of an antibody reaction to the virus.
However, the virus also may be responsible for chronic fatigue in transplant and healthy patients, and occasionally it can cause hepatitis and damage the liver.

News - Epstein Barr Infection Raises Risk of Pediatric Multiple Sclerosis: Presented at AAN
Since seropositivity for Epstein Barr virus is nearly universal by adulthood, any comparative analysis between adult multiple sclerosis patients and controls is limited, she said.
Banwell noted that adults with multiple sclerosis are more likely to be seropositive for Epstein Barr virus than individuals that without the disorder, and that acute mononucleosis in adulthood is known to raise the risk of multiple sclerosis several-fold.
Children offer a better model for studying the association, she explained, since if there is a link, pediatric multiple sclerosis patients would be expected to be Epstein Barr virus-positive at an age when their healthy peers may not yet have had exposure to the virus.
