Failure rate is usually time dependent, and an intuitive corollary is that both rates change over time versus the expected life cycle of a system.
For example, as an automobile grows older, the failure rate in its fifth year of service may be many times greater than its failure rate during its first year of serviceā€”one simply does not expect to replace an exhaust pipe, overhaul the brakes, or have major power plant-transmission problems in a new vehicle.
Failure rates and their projective manifestations are important factors in insurance, business, and regulation practices as well as fundamental to design of safe systems throughout a national or international economy.

Restaurant Failure Rate Much Lower Than Commonly Assumed, Study Finds
A longitudinal study of restaurants in Columbus, Ohio found the failure rate for restaurants was 57 to 61 percent for a three year period (1996-1999) – still high, but much more in line with other businesses, said H.G. Parsa, author of the study and associate professor of hospitality management at Ohio State University.
Cumulative failure rate for the three-year period (1996-99) was 59 percent.
Among franchised chains, the failure rate was 57 percent over the three years and among independent restaurants, the rate was 4 percent higher – 61 percent.

By the way, the "failure" rate wasn't very different between franchised restaurants – 57% — and independent restaurants – 61%.
Now, even though these numbers are much better than the 90% failure rate bandied about on TV, it's not particularly heartening to know that six out of ten restaurants closed in three years.

The study defines failure as "failure to achieve scheduled commercial operations," either because they do not meet their timetable for going online or never go online at all.
According to the report, failure rates varied from 37 percent to 78 percent in projects in northeastern U.S. states such as Massachusetts, New Jersey, New York, and Pennsylvania.
In Europe, failure rates varied from 67 to 78 percent.

The Failure Rate is the rate at which failures occur in a certain time interval for those devices surviving at the start of the interval.
In other words, the Failure Rate Chart FR(t) is proportional to the derivative (rate of change) of the Cumulative Failure F(t) and the Cumulative Survival R(t) charts.
This chart is valuable since the rate of change can be used to infer the percentage of failures attributable to each mode of failure (i.e.

 CCPS - Failure Rate Data
With the advent of Safety Integrity Levels (SIL) representing the relative reliability of instrumented protection systems in a quasi-quantitative manner, there has been a stirring interest by engineers seeking to understand where quality failure rate data can be obtained.
This paper addresses that issue in the context of the Center for Chemical Process Safety (CCPS) initiative regarding operation of an industry Process Equipment Reliability Database (PERD).
The work performed to date regarding instrument loops will demonstrate the overall project, the importance of failure mode definitions and what it takes to obtain high quality trusted data that can be turned into useful information.
