The decreased significance of fascioliasis as a cause of sheep mortality in Debre Berhan in 1986/87 is probably due to the rigorous fluke control measures introduced in March 1986.

Fascioliasis is a parasitic disease of cattle and sheep caused by the liver fluke, Fasciola hepatica.
The rate of fascioliasis was 17% in heifers and steers, 1% in barley beef, 40% in adult cows and 53% in Irish cattle.
These are (a) the strategic use of flukicides in cattle and sheep to reduce pasture contamination with fluke eggs, (b) improved drainage to eliminate the habitats of the snail, and (c) the use of molluscicides to limit snail populations.

In an outbreak of acute invasive fascioliasis in the United Kingdom in 1970, only 20 (71 per cent) of the 28 patients subsequently passed eggs in their faeces, indicating that many flukes perished in other tissues without reaching the bile ducts.
The definitive diagnosis of fascioliasis depends on the identification of the eggs in faeces or duodenal aspirates.
In complicated fascioliasis, abdominal emergencies such as haemoperitoneum and hypovolaemic shock, haemobilia, and cholangitis require a standard surgical approach, including resuscitation with intravenous fluids and blood transfusion, antibiotics, nasogastric tube suction and irrigation, followed by a laparotomy.

As fascioliasis initially involves the liver parenchyma and then the biliary ducts, the disease can mimic most of the common hepatobiliary diseases at different stages of involvement.
Antiparasitic agents used for medical treatment of fascioliasis in the past (e.g., parenteral dehydroemetine and oral bithionol) have not been proven to be very effective.

Human fascioliasis has to be differentially diagnosed from such hepatic and biliary diseases as acute hepatitis, neoplasm, visceral toxocariasis, biliary tract diseases, hepatic amebiasis, and infection with other liver flukes like schistosomiasis [2,5].
As clinical and laboratory findings of fascioliasis may easily be confused with many other conditions, a high index of suspicion is required to establish a correct diagnosis [9,17].
Radiologic manifestations of fascioliasis in a case with peritoneal involvement have been described as low-density lesions in the mesentery [18].

BioMed Central | Full text | Liver, spleen, pancreas and kidney involvement by human fascioliasis: imaging findings
While fascioliasis is a well-known human parasite, it sometimes produces unusual characteristics that may influence a clinician to misdiagnose the condition.
In CT images one can find two distinct kinds of lesions: single or multiple hypodense nodular areas caused by deposition of the parasite (abscess-like lesions) and tunnel-like branching or tortuous hypodensity which is created as the result of parasite migration through the liver and is highly suggestive of the disease [9,17].
Ascites is not a common finding in fascioliasis, but it is not unheard of, as the peritoneum is the usual route of migration of the parasite towards the liver.

 CIGNA - Fascioliasis
Fascioliasis is caused by infection with the parasitic worms of the genus Fasciola, of which “Fasciola hepatica", found in temperate climates, and "Fasciola gigantica", found in tropical climates, are the most common.
Encysted parasitic larvae of these parasites live on water plants, such as watercress, that may be eaten by man or eaten by animals that subsequently are eaten by man. Once ingested, the larvae escape from the cysts in the small intestine and migrate across the intestinal wall into the abdominal cavity.
Fascioliasis is rare in the United States, but sometimes occurs in southern and western areas of the nation where goats and sheep are raised.
