Fasciotomy   (Site not responding. Last check: 2007-10-14)
The most common condition for which fasciotomy is performed is plantar fasciitis, an inflammation of the fascia on the bottom of the foot that is sometimes called a heel spur or stone bruise.
In the case of injury, fasciotomy is done on an emergency basis, and the outcome of the surgery depends largely on the general health of the patient.
Fasciotomy in the limbs is usually done by a surgeon under general or regional anesthesia.

 PES CAVUS SURGERY.
Fasciotomies are designed to reduce the contracture of the plantar fascia that is commonly found in the cavus foot.
The width of excision of a dorsally based transverse wedge of bone extending from the cuboid laterally through the naviculocuneiform joints medially depends on the severity of the deformity to be corrected.
Fasciotomy of the plantar aponeurosis can be performed in conjunction with this procedure if the band is tense.

Surgical Treatment of Heel Spur Syndrome   (Site not responding. Last check: 2007-10-14)
The plantar fascia is a significant stabiliser of the arch, and fasciotomy altered tarsal bone position significantly.
Endoscopic plantar fasciotomy, a recent surgical approach, appears to be frequently indicated for use in America (wysiwyg, 1999).
Given that plantar fasciotomy causes significant arch collapse in cadaver specimens, the clinician should be aware of the importance of surgical indications, and the possibility of consequential instability resulting from the surgery.

plantar fasciitis   (Site not responding. Last check: 2007-10-14)
One journal article says surgery in those with flat feet may not be good (the exact quote is: "The data suggest that operations involving fasciotomy affect arch stability and should not be performed in patients with evidence of concomitant pes planus deformity, because of the likelihood of further deformation").
Although plantar fasciotomies continue to be used to relieve the heel pain associated with plantar fasciitis, there are no criteria as to the amount of fascia release required to relieve symptoms while maintaining the integrity of foot function.
The study demonstrated that partially releasing the plantar fascia imparts forces to the remaining fascia that are not usually observed with the fascia intact.

The Physician and Sportsmedicine: Chronic Leg Pain
Conservative treatment usually consists of reducing the frequency and duration of stressful activities, stretching the involved compartment, using soft orthotic inserts, providing adequate hydration, and using NSAIDs and icing after activity.
Fasciotomy of the deep posterior compartment, release of the attachment of the soleus from the medial tibia, or cauterization of the periosteum is warranted when conservative treatment fails.
Although stress fractures, CECS, and medial tibial stress syndrome are important, the most common cause of leg pain in athletes is musculotendinous strain.
