The fecal fat test measures the amount of fat in the stool, and the percentage of dietary fat that is not taken in by the body.
This test is used to evaluate fat absorption as an indication of how the liver, gallbladder, pancreas, and intestines work.
Normal fat absorption requires bile from the gallbladder (or liver if the gallbladder has been removed), enzymes from the pancreas, and normal intestines.

In medicine, a fecal fat test is a diagnostic test for fat malabsorption conditions (also referred to as steatorrhea).
In the small intestine, dietary fat (primarily triglycerides) is digested by enzymes such as pancreatic lipase into smaller molecules which can be absorbed through the wall of the small intestine and enter the circulation for metabolism and storage.
In the simplest form of the fecal fat test, a random fecal specimen is submitted to the hospital laboratory and examined under a microscope after staining with a Sudan III or Sudan IV dye ("Sudan staining").

We predicted that fecal fat concentration (grams of fat per gram of stool wet weight) would be higher in patients with pancreatic steatorrhea than in those with steatorrhea due to gastrointestinal disease.
To evaluate this hypothesis, we examined fecal weight, fecal fat excretion, and fecal fat concentration in 19 patients with steatorrhea due to pancreatic insufficiency and in 31 patients whose steatorrhea was due to various gastrointestinal diseases.
Fecal fat concentration may be a useful clue in distinguishing pancreatic steatorrhea from steatorrhea due to gastrointestinal disease.

Bile acids are actively absorbed in the terminal ileum and if this enterohepatic circulation is disrupted, fat malabsorption can occur.
If only a small amount of ileum is resected a medication called cholestyramine (Questran, Prevalite) can help improve diarrhea, but if more than 100 cm of ileum has been resected, a low fat diet and medium chain triglycerides (directly absorbed into the circulation without the need for bile salts) is the treatment.
