The only instance in which fel orcs were active on Azeroth was when Mannoroth tricked Grom Hellscream and his warriors into drinking his blood from a tainted fountain of health.
Some believe that the orcs in Warcraft I and Warcraft II were fel orcs too, but not red-skinned, due to their bloodthirst.
A letter recovered from one of the orcs has revealed that Illidan is not pleased with the orcs' progress with the defense of the Blood Furnace.

Orc (Warcraft) - Wikipedia, the free encyclopedia
The orcs do not worship a pantheon of gods, but rather practice a shamanistic religion which venerates the spirits of the elements (Storm, Earth, Fire and Wild, however this was changed later by Blizzard to Water, Earth, Wind, and Fire) as well as the spirits of deceased ancestors.
Orc shamans serve as the spiritual leaders of their clans and draw their power from the elemental spirits, which also helps the shamans to develop an intimate connection with the natural world.

Fel orcs were also encountered by Kael'thas Sunstrider in Outland under the control of the pit lord Magtheridon.

Orc females are smaller than the males, typically around six inches shorter and 50 to 100 pounds lighter.
Despite the belief of the Alliance races that the orcs are uncivilized brutes, the orcs under the leadership of Thrall have clearly managed to establish a complex society with its own distinctive culture.

However, the orcs were later influenced by the Burning Legion and turned into ferocious and merciless warriors.

The orcs of Outland were not the only ones to suffer the Burning Legion's demonic corruption.
The Broken, led by the great Akama, fell prey to the demons' sinister influence, and just like the orcs, they too were changed by the corruption.
Though they lost some of their former powers, the Broken still present a clear danger to all of Illidan's enemies throughout Outland.

The Orcs are savage, brutal, perhaps it's true their society was once Shamanistic as Blizzard puts it....but that does not mean everty Orc was a Shaman.
Orcs still hate the humans, example: Again in War3 ROC in the Orc Campaign (much later in the campaign I believe after they arrived in Kalimdor in one of the chapters, sorry I can't pin point it) one of the Orcs speaks out against the Humans and Thrall talks him down.
Orc expanstion, presence, the reaping of resources from the land and all that for the benefit of the Orcish race....that is what the Orcs are about and portray in thier actions.

Hellfire Citadel - WoWWiki, the Warcraft wiki
Hellfire Citadel is the headquarters of the Fel Orcs of Outland, a nearly impenetrable bastion that once served as the base of operations for the orcs throughout the First and Second Wars.
Its fate after the destruction of Draenor was unknown until recently; it was presumably abandoned until Kargath Bladefist, Warchief of the Fel Horde, and the fel orc minions of Illidan Stormrage took possession of the Citadel as their base of operations.
Illidan also used the citadel as a prison for Magtheridon, the former Lord of Outland; it is here that Illidan's fel orcs take the blood of Magtheridon and use it to create more fel orcs, much as the blood of Mannoroth was used to corrupt the original Horde.

Many have begun to wonder if these unearthly outbursts are somehow connected to the corrupted fel orcs and their growing numbers.
My theory: the fel orcs have Magtheridon chained up in the basement (the raid encounter), and are using his blood to change the remaining orc tribes on Outland into fel orcs.
