Fianarantsoa, bridging high plateaus and the Southeast Coast of Madagascar is one of the poorest of Madagascar’s six newly (2001) autonomous provinces.
In March Fianarantsoa became a strategic focal point in the conflict between the “legal” president and the self-proclaimed “legitimate” president, the latter claiming to have 54% of the vote in the first round and now running a “new” central government in the capital.
Fianarantsoa’s capital on the high plateaus south of Antananarivo was the one provincial capital where the governor had not succeeded in repressing mass demonstrations in favor of Ravalomanana.

As of September 19, the outbreak appeared to be over, with 30,304 cumulative cases and 754 deaths reported from 13 of 111 health districts and four of six provinces (Figure 2); approximately 85% of cases were reported from Fianarantsoa Province.
In Ikongo District (estimated 2002 population: 161,494) of Fianarantsoa Province, the numbers of ARI cases evaluated at health centers and deaths from all causes that occurred during July--August were substantially higher than those that occurred during identical periods in previous years.
Fianarantsoa Province is one of the poorest regions of Madagascar; malnutrition is prevalent, and access to health care is poor.

On 2 March, Olivier Ratsimba and Lalason Rajaobelina, supporters of Marc Ravalomanana in the island of Nosy-Be, part of the northern province of Antsiranana, were separately arrested in their homes and beaten severely by armed security officers and supporters of Didier Ratsiraka.
They were detained in the “2nd RFI” military camp next to the office of the governor, where they were beaten with gun butts and stones, threatened with death, humiliated and robbed of their personal belongings.
Once Ravalomanana’s army retook control of the provinces, many suspected Ratsiraka supporters were arbitrarily arrested or detained by soldiers who do not legally have powers of arrest.

Since then, swarms have been spotted in all the regions in the south and south-west of Madagascar in the province of Toliary and the south of the province of Fianarantsoa.
The situation by province shows a marked fall in harvests in the province of Toliary, which faced the combined effects of drought and locust attacks: 49.5 percent for maize, 17 percent for rice, 13 percent for cassava and 41 percent for sweet potatoes.
The province of Fianarantsoa also suffered locust damage in the southern part which explains the relatively poor results of this season’s rice crop (0.4 percent increase).

In mid-July 2002, Madagascar health authorities were notified of a substantial number of deaths attributed to acute respiratory illness (ART) in the village of Sahafata (population: 2,160), located in the rural highlands of Fianarantsoa Province, southeastern Madagascar (Figure 1).

During July 19-August 22, a total of 152 respiratory specimens were collected for viral isolation from ill persons in three areas of Fianarantsoa Province (Sahafara, Ikongo, and Manandriana) where outbreaks occurred.

Tests on samples taken from victims of the illness in Fianarantsoa province showed a type-A flu virus, said Fadela Chaib, spokeswoman for the World Health Organization (news - web sites).
Fianarantsoa is the poorest province in Madagascar with a high level of malnutrition and a lack of health care, Chaib said.
