  fibrinolysis inhibitor-thrombin topical - [Medication]
Fibrinolysis inhibitor and thrombin are agents that are involved in blood clotting.
Fibrinolysis inhibitor-thrombin topical is used to cause blood clotting during surgery or due to trauma when natural blood clotting processes are deficient.
Fibrinolysis inhibitor-thrombin topical is in the FDA pregnancy category C. This means that it is not known whether it will be harmful to an unborn baby.

Primary Or Secondary Fibrinolysis - Health Centers
Secondary fibrinolysis is the breakdown of blood clots due to a medical disorder,?medicine, or other cause.
The breakdown of fibrin (fibrinolysis) can increase under certain conditions (such as intense exercise, inadequate oxygenation of tissues, low blood sugar, or bacterial infections).
For example, when an abnormal clot forms in the blood vessels of the heart and results in a heart attack, man-made fibrinolytic substances (such as tPA, streptokinase, or Retavase) may be given to break up the culprit clot.

 Fibrinolysis
Fibrinolysis occurs after coagulation, and is the process where a fibrin clot is broken down.
This is released into the blood by the cells of the healthy endothelium[?] neighbouring the clot.
It is important to know if fibrinolysis has recently happened, as this can indicate deep vein thrombosis[?] or a pulmonary embolism.

Fibrinolysis or Angioplasty in Acute Myocardial Infarction - March 15, 2004 - American Family Physician
Rapid angioplasty has been demonstrated to be superior to fibrinolysis in the treatment of patients with acute myocardial infarction; however, immediate angioplasty is available in only a limited number of specialized centers.
Exclusion criteria included any contraindication to fibrinolysis, fibrinolytic therapy in the past 30 days, left bundle branch block, nonischemic heart disease, acute myocardial infarction, pulseless femoral arteries, renal failure, and nonischemic heart disease associated with a life expectancy of less than 12 months.
At presentation to the local hospital, patients were assigned randomly to fibrinolysis with alteplase or transport to a catheterization laboratory for angioplasty.

 ECP
The decision to admit patients to the CCU was made by either the physician or nursing supervisor on duty on the basis of a cursory examination of the patient, findings on electrocardiography performed at admission, or information obtained by ambulance paramedics en route to the hospital.
Although the overall proportion of patients receiving fibrinolysis did not change, there was a trend toward greater use of fibrinolysis among higher-risk patients (who are potentially more likely to benefit from the therapy) after DSCP implementation.
The fact that the DSCP did not change the proportion of appropriate patients receiving fibrinolysis is not unexpected, as it is now standard care to use fibrinolytics in patients with marked ST-segment elevation who present within a few hours of symptom onset.

BioCarta - Charting Pathways of Life
Clot formation and fibrinolysis is a balance of plasmin activation/inhibition and thrombin-thrombomodulin activity that regulates fibrin polymer formation and degradation.
Active thrombin is produced by the cleavage of prothrombin in the intrinsic thrombin activation pathway or the extrinsic thrombin activation pathway.
Thrombin activatable fibrinolysis inhibitor (TAFI) is a carboxy-peptidase B-like proenzyme activated by the thrombin-thrombomodulin dimer.

Dissertations from Karolinska Institutet - Published by Karolinska Institutet Karolinska Institutet - ki.se   (Site not responding. Last check: 2007-10-14)
Accounts of a fibrinolysis inhibitor that is generated by thrombin shed more light on this field and changed the concept by giving thrombin, the pivotal enzyme in hemostasis, another important role in the down-regulation of fibrinolysis.
Thrombin Activatable Fibrinolysis Inhibitor (TAFI), also known as procarboxypeptidase B and procarboxypeptidase U, is a relatively recently described inhibitor of fibrinolysis that can be converted into its activated form which is a carboxypeptidase B- like enzyme, by the thrombin/thrombomodulin complex.
Since this "inhibitor" of fibrinolysis is functional only in its active form, we reserve the term TAFI for the active form of the enzyme and suggest the term TAFli for the inactive form.

EDITOR'S NOTE: An accompanying editorial (1) notes that several other studies have demonstrated that transport for angioplasty is better than locally administered fibrinolysis, in terms of reduced rates of reinfarction and improvements in overall death rates and incidence of stroke.
A combined review (2) of 23 trials comparing fibrinolytic therapy with angioplasty within the same referral center showed death rates decreasing from 9 to 7 percent, reinfarction dropping from 7 to 3 percent, and a slight decrease in stroke from 2 to 1 percent.

 Coagulation and fibrinolysis in sepsis
Thrombin, itself, is an activator of inflammation and inhibitor of fibrinolysis.
The latter is achieved by the activation of TAFI (thrombin-activatable fibrinolysis inhibitor).
The function of this compound is to activate protein C. Activated protein C modifies the inflammatory and coagulant response at several different levels; a deficiency occurs due to inhibition of thrombomodulin in sepsis.

 Euglobulin lysis time
This is one of the best tests to differentiate primary fibrinolysis from DIC (disseminated intravascular coagulation).
The euglobulin lysis test evaluates fibrinolysis (that is, the dissolution of a blood clot).
In primary fibrinolysis (for example, caused by streptokinase administration, cancer of the prostate, shock), the euglobulin lysis time is shorter than normal.
