Folinic acid - Wikipedia, the free encyclopedia
Folinic acid is a 5-formyl derivative of tetrahydrofolic acid.
In this case, folinic acid is not used for "rescue" purposes; rather, it enhances the effect of 5-fluorouracil on inhibiting thymidylate synthase.
Folinic acid is also sometimes used to prevent toxic effects of high doses of antimicrobial dihydrofolate reductase inhibitors such as trimethoprim and pyrimethamine.

Folinic acid is an approved drug used to protect against the side-effects of folate antagonist drugs such as trimetrexate.
Folinic acid needs to be continued for 72 hours after the last dose of trimetrexate to ensure adequate protection.
Folinic acid may also be prescribed for people who are taking other drugs that can interfere with folate metabolism, such as intravenous co-trimoxazole.

 FOLINIC ACID
Folinic acid is a B vitamin that is used to decrease the harmful effects of different drugs, such as pyrimethamine (Daraprim®) or trimetrexate (Neutrexin®).  It is also used to treat certain types of anemias and cancers.
Folinic acid comes as a 5 mg tablet that can be taken with or without food.  It is also available as an injection for intravenous (through your veins) or intramuscular (through a muscle in an arm or buttock) administration.
Folinic acid tablets should be stored in a cool (15-30°C) dry place, protected from light and well out of the reach of children.

 Folate
Folinic acid, also known as leucovorin, citrovorum factor and 5-formyltetrahydrofolate, is used as rescue therapy following high-dose methotrexate in the treatment of osteosarcoma.
For example, 400 micrograms of folic acid taken on an empty stomach is equivalent to 470 micrograms of folic acid taken with food and is equivalent to 800 micrograms of food folate.
Folic acid is indicated for the prevention of some birth defects and appears, as well, to confer significant protection against cardiovascular disease and some forms of cancer.

 theSimonClinic - Psychiatry Without Drugs
This is because ordinarily dietary folic acid must undergo a series of complicated vitamin and energy-dependent changes in the body from the intestine to the liver before it becomes activated to its coenzyme form methylfolate.
Folinic acid available supplementally as oral calcium folinate skips this inefficient transport and streamlining of dietary folates and supplies a ready-made precursor to the brain where it is lacking for a no-nonsense conversion to the coenzyme form methylfolate.
All this makes folinic acid a welcome addition to any psychiatrists tool box because its judicious use as an adjunct to drug therapies will directly increase levels of folate coenzyme where it is lacking and improve the outlook for those where it isn't.

 Folic Acid
It is not surprising that a folic acid deficiency has been implicated in a wide variety of disorders from Alzheimer's disease to atherosclerosis, heart attack, stroke, osteoporosis, cervical and colon cancer, depression, dementia, cleft lip and palate, hearing loss, and of course, neural tube defects.
Folic acid supplementation is known to lower homocysteine levels and laws have recently been passed in the United States mandating folic acid fortification of bread and cereal.
An inadequate intake of folic acid is firmly associated with an increased risk of vascular disease and, among women of childbearing age, with a significantly heightened risk of giving birth to a baby with neural tube defects.

 Product Abstract: Folic Acid
Folic acid supplements in the range of 1 to 2 mg per day are generally innocuous, and usually are sufficient to reduce or normalize high homocysteine levels, even if the elevation is not due to inadequate folic acid supplementation.
At present, 3 approaches to supplementation with folic acid exist: (i) consumption of a diet that is rich in folate and other vitamins; (ii) periconceptional supplementation; and (iii) fortification of food to ensure appropriate folic acid consumption for all women of childbearing age who are capable of becoming pregnant.
Causes of mild increases in plasma homocysteine are usually dietetic deficiencies in folic acid, vitamin B6 or B12, or genetic by mutation of the methylene-tetrahydrofolate reductase.

 Folate (folic acid) - MayoClinic.com
Folic acid is well-tolerated in amounts found in fortified foods and supplements, in doses less than 1,000 micrograms per day.
Doses of at least 400 micrograms folic acid per day from supplements or fortified food should be taken by women planning to become pregnant and continued through the first month of pregnancy.
Folic acid supplementation in doses higher than 0.8 milligrams (800 micrograms) daily is categorized as FDA Pregnancy Category C. Based on human study, folic acid appears safe to use in pregnant women in a dose of 500-1,500 micrograms per day without lessening the protective effects of sulfadoxine/pyrimethamine as an intermittent preventative treatment for malaria.

Folic acid and folinic acid for reducing side effects in patients receiving methotrexate for rheumatoid arthritis
To assess the effects of folic acid and folinic acid in reducing the mucosal and gastrointestinal (GI) and haematologic side effects of low-dose of Methotrexate (MTX) in patients with Rheumatoid Arthritis (RA) and to determine whether or not folate supplementation alters MTX efficacy.
No consistent differences in disease activity parameters were observed when comparing placebo and folic or folinic acid at low or high doses, although patients on high dose folinic acid had an increase in the number of tender joints, but not swollen joints.
Therefore, for folinic acid to be considered cost-effective it must be found more effective than folic acid at reducing MTX side effects.

Methotrexate is a folate antagonist and supplementation of MTX–treated patients with folate or folinic acid is believed to reduce MTX–associated toxicities.
Patients who were naïve to MTX were begun on MTX 7.5 mg/wk and were randomized to folate 1 mg/day OR folinic acid 2.5 mg/wk OR placebo.
Mean dosages of MTX at the end of the study were lower in the placebo group (14.5 mg/wk) than in the folic and folinic acid groups (18.0 and 16.4 mg/wk, respectively).

Folinic acid injection can be used to reduce the side effects from high doses of the anticancer medicine methotrexate.
It can also be used in combination with the anticancer medicine fluorouracil (5-FU), as it has been shown to boost the response rate to this anticancer medicine.
Folinic acid is used in combination with 5-FU to increase the amount of tetrahydrofolate inside the cells and therefore boost the effect of the 5-FU.
