In anatomy, the foramen magum is the large hole through the occipital bone in the base of the skull, through which the medulla oblongata (an extension of the spinal cord) exits the skull vault.
In humans the foramen magum is farther underneath the head than in great apes.
Comparisons of the position of the foramen magum in early hominid species are useful to determine how comfortable a particular species was when walking on two limbs (bipedality) rather than four.

The curved, expanded plate behind the foramen magnum is named the squama; the thick, somewhat quadrilateral piece in front of the foramen is called the basilar part, whilst on either side of the foramen is the lateral portion.

This foramen is present in the squamous part of the occipital bone at the occipital protuberance and it transmits a vein that connects the confluence of the sinuses with the veins of the suboccipital plexus and receiving also the occipital diploic vein.
We have examined 150 dry adult human skulls to identify the positions of the emissary foramen in the squamous part of the occipital bone either left or right side of the median plane, in the median plane from the posterior margin of the foramen magnum to the occipital protuberance.
In each case the patency of the foramen was confirmed with a probe and measurements in relation to the posterior margin of the foramen magnum and from the median plane were measured by millimeter scale.
