 CATHOLIC ENCYCLOPEDIA: Censorship of Books
Regarding them paragraph 11 says: All books are forbidden that insult God or the Blessed Virgin Mary or the saints or the Catholic Church and her rites, the sacraments or the Apostolic See.
Under the same penalty, and in like manner, books individually condemned by letters Apostolic are indicted by paragraph 47, in case the letters referred to are still in full force, and punish the reading of the condemned book with excommunication reserved to the pope.
From the previously mentioned arrangement of all forbidden books in three groups it clearly follows that the Church not only keeps within the limits of her right, but also forbids only as much as she is bound to forbid by reason of her office as teacher and guide of all the faithful.

Catholic Culture : Document Library : The Index Of Forbidden Books
There are a thousand books forbidden by the rules of the Index for every one that is nominally mentioned in its specific list; and it is the spirit of the Index to protest against every bad book, whether named or not, that can harm the minds and hearts of the faithful.
Such books are calculated to corrupt minds that are too immature to form an independent judgment; and, whatever may have been the intention of their writers, the Church, as the guardian of her children, is bound to warn these against such contamination.
There is, however, a large and growing number of books which treat of religious subjects and which are written by non-Catholics who admire the stoic moral philosophy of men like Plato or Mark Aurelius, because their doctrine makes for the cultivation of the natural virtues of truthfulness, kindness, purity, thrift, and justice.

Books which deal in the vernacular with the controversies between Catholics and heretics of our time may not be permitted indiscriminately, but the same is to be observed with regard to them what has been decreed concerning Bibles written in the vernacular.
Books whose chief contents are good but in which some things have incidentally been inserted which have reference to heresy, ungodliness, divination or superstition, may be permitted if by the authority of the general inquisition they have been purged by Catholic theologians.

CATHOLIC ENCYCLOPEDIA: Index of Prohibited Books
ENSORSHIP OF A book was prohibited or put on the Index by decree of the Sacred Congregation of the Roman Inquisition, of the Sacred Office, or of the Index, which decree though approved by the pope (in formâ communi), always remained a purely congregational decree.
Together with the report of the first revisor — who remained anonymous — the book was given to another for a second revision, and only when the second revisor's verdict was in agreement with that of the first were both reports referred to the cardinals for final decision.
The first Roman "Index of Prohibited Books" (Index librorum prohibitorum), published in 1559 under Paul IV, was very severe, and was therefore mitigated under that pontiff by decree of the Holy Office of 14 June of the same year.

Because Lima was a very important market of books it was difficult to adequately censor it, the control of the diffusion of forbidden books was not always possible for the Inquisition.
