If the game ends with both players having their Ta'veren/Forsaken killed before the Last Battle, they both count the average of the three highest Tie Breaking Point totals for the game (from all players, both Hero and Villain) as negative Tie Breaking Points.
For the fourth Game of the tournament, once again sort the players into Hero and Villain piles, and match them up again as explained in the second Game of the first cycle (remember to use a player's total score, and not just his score from this Game).

Rahvin, as can be expected of a Forsaken, greatly lusts for power (The Shadow Rising, Chapter 58) According to Asmodean, Rhavin only looks after his own interests, and would sit on the sideline, even during the Last Battle (The Fires of Heaven, Chapter 6).

I read the Rand/Rahvin thing, as Rahvin (as some of the other forsaken do), saw his old enemy LTT instead of Rand, and so was imagining his body rather than Rands own which may have caused the change.
The Forsaken don't seem to have any problem appearing in T'A'R for chats, or going there in the flesh as Ishy and Rahvin did in their battles with Rand.

Her lack of past almost reminds me of how the newly released Forsaken just dropped into major player roles in the Council of Nine, as Tuon's Truth Speaker, etc. I don't suggest she's one of them, but if the Creator were going to drop in anti-Forsaken somewhere, she'd fit the bill.
I also wonder if she's a reincarnation of the last Queen of Manetheren (Ellisande?) She and Egwene both somehow just seem to "remember" how to do some weaves and pick up stuff really fast.

AKA Ba'alzamon, AKA Moridin Ishamael, who eventually becomes the head of the Forsaken, was a rival of Lews Therin Telamon who felt that he would be the premier man of the age if not for LTT.
He was always a rival of Lews Therin Telamon and was envious of Lew's widespread fame and notoriety.
He, like all the other Forsaken, was sealed in the Bore by Lews Therin and the Hundred Companions when they made their strike at Shayol Ghul.

This didn't really answer the question of how the taint got there in the first place (or whether or not it will come back), but I would like to point out that while most of the Forsaken showed up at Shadar Logoth to try and stop Rand, Moridin was conspicuous in his absence.
Ishamael cleansed part of the saidin's taint from Lews Therin's body and soul, lifted the taint's madness from Kinslayer's mind so he could understand what had really happened.
The Forsaken calls it 'Shai'tan's healing' (and I thought calling the Dark One by his true name was blasphemy - what's up with that, Elan Morin?), and such a name is well-earned.
