In a fountain, each hand juggles separately, and the objects are not thrown between the hands.

JUGGLING THEOREM proposed by Claude E. Shannon of the Massachusetts Institute of Technology is schematically represented for the three-ball cascade.
The fountain pattern, in contrast, can be stably performed in two ways: by throwing (and catching) the balls simultaneously with both hands (in sync) or by throwing a ball with one hand and catching one with the other at the same time (out of sync).
Bounce juggling is easier to accomplish than is toss juggling because the balls are grabbed at the top of their trajectories, when they are moving the slowest.

Throughout this paper the diagrams of juggling patterns have throw and catch positions labeled T and C. The path of the hand between these positions is represented by a dotted line; the exact path is unspecified.
Because the throws in the fountain are aimed toward the outside of the pattern (unlike the cascade where throws are made towards the middle) the hand must reverse the horizontal velocity of the ball twice (in a cascade the hand reverses the horizontal velocity of the ball only once).

If you can juggle two balls in your right hand, and juggle two balls in your left hand, then you are able to do 4 ball juggling, which is just doing both tricks at the same time!
In 4 ball juggling, the balls do not change hands (until you are ready for some more advanced tricks!) The same rule applies for any even number of objects you care to juggle (eg 6 balls, 8 clubs etc).
You are still juggling two balls in each hand, but throwing from each hand at a different time.

 Re: Technique for ring pulldown?
On occasion I bonk my ears or nose, but I think the reason I pull down a ring on the side as opposed to from back to front is that the sideways approach doesn't disrupt the positioning of the arms during the pulldown.
It would seem that from the back to front approach, you would catch the ring, then move the positioning of your arm from either the cascade or fountain you are juggling towards the back of your head.
With breaking down from the side (just in my opinion), the ring is caught from the pattern, and then immediately placed around your neck from the side.
