The clinical usefulness of serum gamma-glutamyl transpeptidase (gamma GT) assay for the diagnosis of liver disease in children was assessed retrospectively in 398 children investigated from 1981 to 1986, in whom diagnosis was ascertained according to currently accepted criteria including liver histology in each case.
Serum gamma GT was raised in all children with biliary atresia, sclerosing cholangitis, paucity of interlobular bile ducts, and alpha 1-antitrypsin deficiency with jaundice.
Serum gamma GT was normal in spite of patent clinical signs of cholestasis in 3 patients with benign recurrent intrahepatic cholestasis, 1 infant with post-hemolytic neonatal cholestasis, and in 22 of 28 patients with progressive idiopathic cholestasis akin to Byler disease.

GGT is a biliary excretory enzyme which is more specific for hepatic disease than is alkaline phosphatase.
When GGT and alkaline phosphatase are both high, but one is disproportionately elevated, suspect the possibility of drug-induced cholestasis (including alcoholism if it is GGT which is much higher).
GGT is normal in normal children, adolescents, and in pregnant women.

A good example for such an enzyme is -glutamyl transpeptidase (GGT) which is induced to high levels in the very early stages of liver and skin cancer.
GGT participates in a metabolic pathway which detoxifies and excretes carcinogens, thus allowing GGT-rich pre-cancerous cells to survive in a carcinogen-induced toxic environment, whereas GGT-poor cells cease to proliferate or die out.
GGT may participate in the elevation of glutathione concentration within cells, thus increasing their resistance to acute toxicity and facilitating their proliferation.
