Garth of Izar is a fictional character in the universe of Star Trek who was a starship captain who achieved great fame in the mid 23rd century.
A native of the planet Izar, Garth was a Fleet Captain in Starfleet before being relieved of his command after suffering from a severe mental illness resulting from a terrible physical injury.
Garth was eventually halted in his plans by Captain James T. Kirk and returned to the custody of the mental hospital from which he had escaped.

This book, by two respected SF writers, jumps forward a few years to when a miracle drug has cured Garth and other asylum inmates of their madness and Garth is once again acting captain and on a mission, but under the watchful eyes of Kirk and his crew.
Their mission is to return to Antos IV and attempt to diffuse the crisis which is threatening to erupt into a civil war, as the shape shifting Antosians are divided on whether to lose their now-useless skill in order to better serve as a member of the Federation or to keep their heritage intact.
At times I felt that the story and character of Garth demanded a bit of Shakespearean tragedy, but the authors had obviously thought hard about what was most likely to happen if the situation had been real, and when I put the book down I felt that their decision had been for the best.

For the record, while Steve Ihnat played the character of "Garth of Izar", it was Lee Erwin and Jerry Sohl who conceived him and without them he simply wouldn't exist.
Antos IV is a planet of shape-shifters and after Garth sustained his injuries the tried to bring Garth back to his former health, but now he has shape-shifting abilites and the cure has proved to be worse than his injuries driving Garth to insanity.
Captain Garth is sent to Elba II due to the madness that the Antosians inadvertinatly passed to Garth while being treated.

The article on the penal colony on Elba II indicates that it's a high-security complex for dangerous criminals, as does the article on the planet Elba II itself.
This article on Garth, however, specifically states that the penal colony on Elba II is low-security.
So it might be more comfortable looking than a prison, but between the poisionous atmosphere surrounding it, and the sheilds that Enterprise can't dent, it's pretty high-security to me. I think the perspective might be wrong, anything could be low-security from the POV of incarcerating a shapeshifter.

Garth was a starship captain and had a hand in the Axanar conflict if I'm remembering right.
He also said Garth was one of his heroes and reminded Garth how everyone at the Academy looked up to him and admired him.
Garth briefly admitted that it was a huge burden to carry but he was proud to do so.

I found the storyline to be captivating and the reintroduction of Garth to be a strong character to balance the character of Kirk.

McCoy mentions his daughter Joanna: the first on-screen mention of such a thing although his daughter was referenced in the original Trek script bible.
This is the second time that we saw a race of alien shapeshifters (the first being Garth of Izar in original Trek's "Whom Gods Destroy", who was taught how to shapeshift by the Izarites).
This makes later references about the Federation's unfamiliarity with shapeshifters (Odo is described as unique, and shapeshifters are described as "legendary" in Star Trek VI: The Undiscovered Country.) Banishing this episode from canon doesn't explain the Garth of Izar thing.

Surely there are any number of things Spock could have done when faced with the two Kirks.
For instance, he could have stunned both of them and then had a hundred security personnel beam down from the ship to deal with the problem while he sorted Garth and Kirk out at his leisure.
