eMedicine - Genioplasty : Article Excerpt by: Edward W Chang, MD, DDS
While patients seeking advice about facial cosmetic surgery often focus on structures such as the nose, the eyes, and the laxity of their skin, the facial plastic surgeon's assessment frequently identifies the lower third of the face as an area that could be surgically modified to improve overall facial appearance and harmony.
However, to debate whether alloplastic augmentation or osseous genioplasty is the superior choice is beyond the scope of this article.
In general, genioplasty implies an osseous movement, whereas mentoplasty suggests the use of an alloplastic implant.

Genioplasty - Cosmetic Procedures - Barry M. Jones MS FRCS
Most commonly, the procedure is that of chin augmentation to give a stronger profile, but it is possible to reduce the chin both in its profile, contour and also in its vertical height as well as augmenting it and correcting asymmetries.
It is not uncommon for patients to complain that their nose is too prominent when, although this may be true, the situation is exacerbated by a relatively small chin.
There are two main approaches to augmentation genioplasty: either using a prosthetic (artificial material) to make the chin more prominent and/or longer, or adjusting the position of the patient's own bony chin to change the contour.

Genioplasty - New York Presbyterian Hospital
Most people who decide to have genioplasty are seeking to balance the appearance of their facial features by enlarging their chin and increasing its projection on the face, though in some cases genioplasty is also done to make the chin smaller.
It is not unusual for genioplasty to be done in combination with other surgical procedures to the face, such as orthognathic jaw surgery or rhinoplasty.
In this latter type of genioplasty, the tip of the chin is cut from the lower jaw and physically moved forward.

 Genioplasty (Chin Surgery)
A genioplasty is a surgical procedure carried out to reshape or change the size of the chin.
Using specialized instruments, the bone of the chin is reshaped to balance your profile in relationship to the nose, jaws and lips.
A genioplasty may have dramatic positive affects on your profile and shape of the lips.

Chin enhancement (or “genioplasty”) is a procedure intended to correct chin position either through bony surgery or placement of a small implant.
Patients often seek a genioplasty as part of a plan to improve the balance, symmetry and proportion of their lower face.
The genioplasty is usually combined with other procedures such as face lift or nose reshaping (“rhinoplasty”).

Surgery may be performed in the surgeon’s office-based facility, a hospital, or an outpatient facility.
The best candidates for genioplasty are patients with weak or receding chins (microgenia), but with a normal dental bite.
Expectations of looking and feeling better after surgery should be discussed with the plastic surgeon before any surgery, keeping in mind that the desired result is improvement, not perfection.

The ratios of distance from subnasale to upper lip stomion, upper lip stomion to menton, and the amount of skeletal and soft tissue advancement were calculated from lateral cephalograms.
Results: Mean lateral translation of the genioplasty segment was 10 mm (range, 5 to 12 mm) and the angle of rotation was 4º (range, 2 to 6º).
Conclusion: Extended lateral sliding genioplasty is a versatile technique for the correction of facial asymmetry and retruded chin due to unilateral mandibular deficiency.

Genioplasty is a surgical procedure in which the projection or the length of the chin is changed.
This is a more technically demanding procedure but often leaves the patient with a more natural appearance (as opposed to the look of the "done" chin) and avoids the need for prosthetic implants.
It is not unusual to combine genioplasty with rhinoplasty or orthognathic surgery to improve the profile.

 Re: Genioplasty
Genioplasty seems to be very similar to a genioglossus advancement (GA) in that they both help pull the genioglossus muscle/tongue forward.
My understanding is that with genioplasty the tip of the chin bone is sliced and pulled forward.
Genioplasty might be more preferrable if you also desire cosmetic enhancements for a small chin.

 Chin Augmentation - Aesthetics - Dukehealth.org
Chin surgery, also known as genioplasty, is a surgical procedure to reshape the chin by enhancement with an implant.
Many times a plastic surgeon may recommend chin surgery to a patient having nose surgery in order to achieve facial proportion, as the size of the chin may magnify or minimize the perceived size of the nose.
For a so-called “sliding genioplasty,” a sliver or section of bone is "slid" forward.

Having long teeth with a short mandibular height is a relative contraindication for an osseous genioplasty or an aggressive bony reduction.
The sliding genioplasty can also be performed under local anesthesia or in an outpatient setting with good results; however, general anesthesia is most commonly used with this procedure.
Complications observed with genioplasty are minimal, and benefits are readily evident to both patient and surgeon.

In addition to improving the appearance of your chin and facial proportion, mentoplasty / genioplasty can also improve your neckline by sharpening the angle between your chin and neck - thus tightening a neckline and reducing the "double chin" effect of a receded chin.

Alloplastic augmentation is typically reserved for older persons with minimal pure horizontal chin deficiencies, and osseous genioplasties are performed in younger patients with more pronounced anomalies.
Interpositional genioplasty, which is indicated for correction of vertical deficiency alone, is accomplished by performing a horizontal osteotomy with interpositional grafting using bone or a bone substitute.
Reduction genioplasty, which involves wedge ostectomy followed by securing of the caudal segment to the mandible, is used for correction of vertical excess alone.
